吴鞠通治疗温疫用药规律探析

摘　　要：目的:基于R语言,对《吴鞠通医案》中治疗温疫的方剂进行数据挖掘,分析吴鞠通治疗温疫的用药规律。方法:以《吴鞠通医案》中“温疫”篇的医案为研究对象,采用Excel 2021对处方用药信息进行整理,包括单味药使用频次、性味、归经、功效;并运用R语言(V4.2.3)、R Stduio平台软件对录入的数据进行频数频率分析、聚类分析和关联规则分析。结果:本研究共采集到处方127首、涉及中药105味、使用总频次为1398次。高频药物有生地黄、麦冬、甘草、玄参、牡丹皮等。高频药物药性以寒性为主,药味以甘味和苦味居多;药物归经主要涉及肺、胃、肝、脾经;功效以清热、化痰、解表为主。关联规则分析得到生地黄-麦冬、麦冬-牡丹皮-生地黄等核心药对;通过聚类分析,得出4组核心药物组合。结论:吴鞠通治疗温疫多从三焦辨治,灵活应用凉泻或清泻之法,且重视顾护脾胃,重点在于解毒,彻底清除疫毒。Objective:Based on R language,the data mining of prescriptions for the treatment of epidemic diseases in WU Jutong Medical Cases was carried out to analyze the medication rules of WU Jutong in the treatment of epidemic diseases.Methods:The medical records of epidemic diseases in WU Jutong Medical Cases were taken as the research subjects,Excel 2021 was used to sort out prescription drug information,including the use frequency,property and flavor,channel tropism and efficacy of single medicinal.And R(4.2.3)and R Stduio platform software were used to analyze the frequency,cluster and association rules of the input data.Results:A total of 127 prescriptions were collected in this study,involving 105 Chinese materia medica,and the total frequency of use was 1398 times.The high-frequency drugs were:Shengdihuang(Radix Rehmanniae Recens),Maidong(Ophiopogonis Radix),Gancao(Glycyrrhizae Radix Et Rhizoma),Xuanshen(Scrophulariae Radix),and Mudanpi(Moutan Cortex),etc.The high frequency drugs were mainly cold,sweet and bitter.It mainly involved lung,stomach,liver and spleen channels.The main effects were clearing heat,eliminating phlegm and relieving surface.The core drug pairs such as Shengdihuang-Maidong,Maidong-Mudanpi-Shengdihuang were formed by association rule analysis.Through cluster analysis,four core drug combinations could be obtained.Conclusions:WU Jutong differentiated and treated epidemic diseases mainly from sanjiao,flexibly applied the method of cool dredging or clear dredging,and paid attention to the spleen and stomach of middle jiao.The key was detoxification,and the epidemic evils should be completely removed.
