摘　　要：张志远先生治疗胸腔积液善用经方而不泥,善于巧用、化裁、创新。认为饮邪与“痰、热、滞、瘀、积”既可互为因果,也可彼此胶结加重病情,在逐饮同时要兼顾它们之间的关系加以处理。治疗上活用十枣汤、大陷胸丸等逐饮峻泻之剂,又依据临床经验,创制三一汤、祛饮荡水汤、开胸五将、三合一汤、十位耆婆等功效稳妥、平而含奇的药方,组方严谨,各有侧重点,为临床治疗胸腔积液提供了宝贵经验。Master ZHANG Zhiyuan is skilled in applying classic prescriptions into the treatment of pleural effusion,and is also good at adding and subtracting,modifying as well as innovating the prescriptions.It is believed that fluid retention can lead to phlegm,heat,stagnation,stasis and indigestion,and vice versa,and their combination may exacerbate the condition.Therefore,it is necessary to balance the relationship between them while expelling fluid retention.In the treatment,Master ZHANG makes flexible use of prescriptions with drastic purgative effect for expelling fluid retention,including Shizao Decoction(十枣汤)and Daxianxiong Pill(大陷胸丸).Based on clinical experience,he creates many unique prescriptions with reliable and unique efficacy,such as Sanyi Decoction(三一汤),Quyin Dangshui Decoction(祛饮荡水汤),Kaixiong Wujiang(开胸五将),Sanheyi Decoction(三合一汤)and Shiwei Qipo(十位耆婆),etc.,which are rigorous in prescriptions and have specific focus in the treatment and provide valuable experience for the clinical treatment of pleural effusion.
