摘  要:目的:从菌群总体角度分析和比较高强度聚焦超声(HIFU)治疗子宫肌瘤前后患者阴道微生态的变化,探讨HIFU治疗子宫肌瘤对患者阴道微生态的影响。方法:选取10例因单发子宫肌瘤接受HIFU治疗的女性入院时的阴道分泌物为对照组,同时采集10例在HIFU治疗后1个月的阴道分泌物为实验组。提取菌群总基因组DNA,扩增16S rRNA V4区基因片段后测序,利用生物信息学分析技术分析20例样本的微生物组成及微生物菌群差异。结果:基于菌门级别分类,文库内所有序列分属10大门,厚壁菌门在两分组文库中的数量都是最多,其中乳酸杆菌为优势菌属,HIFU治疗前后阴道菌群结构未见显著差异,而以个体为特点区分聚类;α多样性分析表明HIFU治疗前后α多样性无显著差异(P>0.05);β多样性分析表明HIFU治疗前后两组生殖道细菌谱有广泛的相似性和重叠性;对照组与实验组之间物种总数及微生物多样性无显著差异(P>0.05);ANOSIM相似性分析展示组间无统计学差异(P>0.05)。结论:HIFU治疗子宫肌瘤不会破坏患者阴道微生态,具有较高的安全性及良好的预后,能有效改善患者生活质量降低并发症发生率,可成为子宫肌瘤新的重要治疗模式,值得临床推广与应用。Objective:To analyze and compare the changes in vaginal microbiota before and after High Intensity Focused Ultrasound(HIFU)treatment of uterine fibroids from a microbial community perspective,and to preliminarily elucidate the impact of HIFU treatment on the vaginal microbiota of patients with uterine fibroids.Methods:Vaginal secretions were collected from 10 female patients who were admitted to our hospital for solitary uterine fibroids and treated with HIFU,both at the time of admission and 1 month after HIFU treatment.Total microbial genome DNA was extracted,16S rRNA V4 region gene fragments were amplified and sequenced,and bioinformatics analysis techniques were used to analyze the microbial composition and the differences in microbial communities in 20 samples.Results:Based on phylum-level classification,all sequences in the library belonged to 10 phyla,with Firmicutes being the most abundant in both groups,and Lactobacillus was the dominant bacterium.There was no significant difference in the variaty of vaginal microbiota before and after HIFU treatment,with clustering distinguished by individual characteristics.αdiversity analysis showed that there was no significant difference in alpha diversity before and after HIFU treatment(P>0.05).βdiversity analysis showed a wide similarity and overlap in the reproductive tract bacterial spectrum before and after HIFU treatment.There was no significant difference in the total number of species and microbial diversity between the control group and the experimental group(P>0.05).ANOSIM similarity analysis showed no significant difference between groups(P>0.05).Conclusion:HIFU treatment demonstrates minimal impact on the vaginal microbiota in patients with uterine fibroids,thereby ensuring a high level of safety and favorable prognosis.Furthermore,this treatment has shown efficacy in enhancing patient quality of life and reducing the occurrence of complications.Consequently,HIFU treatment holds promise as a significant and valuable therapeutic approach for uterine fib
