血清Es、Ps、NLR在肺炎支原体肺炎患儿表达水平及判断近期不良预后的价值研究

摘　　要：目的探讨肺炎支原体肺炎(MPP)患儿血清中E-选择素(Es)、P选择素(Ps)及中性粒细胞/淋巴细胞比值(NLR)表达水平,并分析Es、Ps及NLR联合检测在判断患儿不良预后中的价值。方法回顾性连续纳入邢台医学高等专科学校第二附属医院2019年1月—2021年1月期间收治的100例MPP患儿,收集其临床资料,根据病情的严重程度分为轻度组(n=49)和重度组(n=51)。比较不同病情严重程度患儿血清Es、Ps、NLR水平;记录不良预后发生率,观察预后不良组、预后良好组血清Es、Ps、NLR变化;通过受试者工作特征(ROC)曲线评估Es、Ps及NLR判断MPP患儿不良预后的价值。结果重度组患儿血清NLR、Es及Ps水平均高于轻度组(t=7.717、8.271、15.362,P<0.05)。100例患儿中,不良预后16例,不良预后发生率16.00%;预后不良组Es、Ps及NLR均高于预后良好组(t=4.252、6.611、5.270,P<0.05),预后不良组病情重度占比高于预后良好组(χ^(2)=10.155,P<0.05)。经ROC曲线分析,Es+Ps+NLR联合检测判断MPP不良预后的曲线下面积(AUC)为0.955(95%CI:0.894~0.986,P<0.001),敏感度和特异度分别为93.75%和84.52%。结论血清Es、Ps以及NLR水平与MPP患儿病情严重性相关,且Es、Ps及NLR联合检测对判断MPP患儿近期不良预后具有较高的准确性。Objective To study the expression level of E-selectin(Es),P-selectin(Ps)and neutrophil/lymphocyte ratio(NLR)in serum of children with mycoplasma pneumoniae pneumonia(MPP),the value of combined detection of Es,Ps and NLR in judging the poor prognosis of children was analyzed.Methods The 100 children with MPP admitted in the Second Affiliated Hospital of Xingtai Medical College from January 2019 to January 2021 were included retrospectively and continuously,and the clinical data of them were collected.The children were divided into mild group(n=49)and severe group(n=51)accorded to the severity of the disease.The levels of serum Es,Ps and NLR in children with different severity of illness were compared.The incidence rate of poor prognosis was recorded,and the changes of serum Es,Ps,and NLR in groups with poor and good prognosis were observed.The value of Es,Ps and NLR in judging poor prognosis in children with MPP was evaluated through receiver operating characteristic(ROC)curve.Results The levels of serum NLR,Es and Ps in the severe group were higher than the mild group(t=7.717,8.271,15.362,P<0.05).There were 16 cases had poor prognosis among 100 children,and the poor prognosis rate was16.00%.The Es,Ps and NLR in the poor prognosis group were higher than the good prognosis group(t=4.252,6.611,5.270,P<0.05),and the proportion of severe illness in the poor prognosis group was higher than the good prognosis group(χ^(2)=10.155,P<0.05).Through ROC curve analysis,the area under the curve(AUC)of Es+Ps+NLR joint detection for judging poor prognosis of MPP was 0.955(95%CI:0.894-0.986,P<0.001),and the sensitivity and specificity were 93.75%and 84.52%respectively.Conclusion The level of serum Es,Ps and NLR are correlated with the severity of MPP in children,and the combined detection of Es,Ps and NLR has high accuracy in judging the recent poor prognosis of children with MPP.
