老年人手部猫、犬咬伤Ⅲ级暴露的伤口感染情况分析

摘　　要：目的研究老年人手部猫、犬咬伤Ⅲ级暴露的伤口感染情况。方法选取北京市海淀医院急诊外科2020年1月—2023年6月就诊的手部猫、犬咬伤Ⅲ级暴露老年患者236例,根据致伤动物的不同将其分为A组(112例)和B组(124例)。A组为猫咬伤,B组为犬咬伤。比较两组的感染情况。结果B组的感染率低于A组(χ^(2)=9.710,P<0.05)。A组感染最快发生在清创后1 d,清创后3 d感染率达高峰,之后感染率逐渐下降。但两组清创1、2、3、4、5 d感染率比较差异无显著性(P>0.05);B组感染最快发生在清创后2 d,清创后3 d感染率达高峰,之后感染率逐渐下降。B组感染仅局限于手部的感染率高于A组(χ^(2)=5.539,P<0.05)。结论老年人手部猫咬伤Ⅲ级暴露伤口相较于犬咬伤Ⅲ级暴露伤口感染发生的时间更早,感染速度更快,感染程度更重,感染率更高。Objective To study wound infection situations in gradeⅢexposure of hands caused by cats and dogs bites in elderly people.Methods Two hundred and thirty-sixelderly patients with gradeⅢexposure of hands caused by cats and dogs bitestreated in Department of Emergency Surgery of Beijing Haidian Hospitalfrom January 2020 to June 2023 were selected and divided into group A of 112 cases and group B of 124 cases based on different animals causing injuries.Group A caused bycat bites,group B caused bydog bites.Infection status between two groups was compared.Results Infection rate of group B was lower than that of group A(χ^(2)=9.710,P<0.05).Fastest infection in group A occurred one day after debridement,and infection rate peaked at three days after debridement,after which infection rate gradually decreased.Fastest infection rate in group B occurred two days after debridement,with highest infection rate at three days after debridement,and infection rate gradually decreased thereafter However,there was no significant difference in the infection rate between the two groups on the 1^(st),2^(nd),3^(rd),4^(th) and 5^(th) day of debridement(P<0.05).Infection rate of infection limited to hands only in group Bwas higher than that in group A(χ^(2)=5.539,P<0.05).Conclusion Infection of gradeⅢexposure of hands caused by cats in elderly peopleoccurs earlier,faster,more severe,and has a higher infection ratethan those caused by dogs.
