摘　　要：目的探讨阿加曲班联合倍他司汀治疗椎基底动脉供血不足眩晕(VBIV)的效果及对血流状态的影响。方法取2016年10月至2021年8月期间,收治的150例椎基底动脉供血不足眩晕患者,经随机数字表法分组,对照组采取常规治疗加倍他司汀,观察组在对照组基础上加阿加曲班,比较两组治疗效果。结果治疗总有效率比较,观察组明显较对照组高(P<0.05);与对照组治疗后眩晕评定量表(DARS)、眩晕障碍量表(DHI)评分比较,观察组明显更低(P<0.05);与对照组治疗后血流速度比较,观察组明显更高(P<0.05);两组不良反应发生率无明显差异(P>0.05),观察组短暂性脑缺血发作(TIA)发生率较对照组低(P<0.05)。结论阿加曲班联合倍他司汀治疗VBIV的效果满意,可有效改善眩晕症状,提高椎基底动脉血流速度,且安全性高,还能降低TIA发生率,值得推广应用。Objective To explore the effect of Agatroban combined with Betamethasone in the treatment of vertebrobasilar insufficiency vertigo(VBIV)and its impact on blood flow status.Methods From October 2016 to August 2021,150 patients with vertigo due to insufficient blood supply to the vertebral basilar artery were randomly divided into two groups using a random number table method.The control group received routine treatment with double dose of statin,while the observation group received additional treatment with agatroban on top of the control group.The treatment effects of the two groups were compared.Results The total effective rate of treatment was significantly higher in the observation group than in the control group(P<0.05);compared with the control group,the observation group had significantly lower scores on the Posttreatment Dizziness Rating Scale(DARS)and Dizziness Disorder Inventory(DHI)(P<0.05);compared with the control group,the observation group had significantly higher blood flow velocity after treatment(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05),while the incidence of transient ischemic attack(TIA)in the observation group was lower than that in the control group(P<0.05).Conclusion The combination of Agatroban and Betamethasone has a satisfactory effect on the treatment of VBIV,which can effectively improve dizziness symptoms,increase vertebral basilar artery blood flow velocity,and has high safety.It can also reduce the incidence of TIA,and is worthy of promotion and application.
