摘　　要：目的探究右美托咪定静脉泵注在腰-硬联合麻醉剖宫产术中的应用效果及对应激反应的影响。方法选2022年7月~2023年9月我院接诊剖宫产孕产妇104例,以随机数字表法,划分成对照组、观察组。对照组麻醉方案:腰-硬联合麻醉,观察组麻醉方案:右美托咪定静脉泵注+腰-硬联合麻醉,对比两组应用效果、应激反应及血流动力学情况。结果观察组在切口缝合时(T2)、术后2h(T3)阶段Ramsay评分高于对照组,视觉模拟评分(VAS)低于对照组(P<0.05);观察组在T2、T3阶段皮质醇(Cor)、去甲肾上腺素(NE)、C反应蛋白(CRP)水平均低于对照组(P<0.05);观察组在T2、T3阶段平均动脉压力(MAP)、心率(HR)均高于对照组(P<0.05)。结论右美托咪定静脉泵注在腰-硬联合麻醉剖宫产术中麻醉效果显著,可缓解应激反应,维持机体血流动力学稳定,值得推广。Objective To investigate the application effect of intravenous infusion of dexmedetomidine in combined lumbar epidural anesthesia for cesarean section and its impact on stress response.Methods 104 pregnant women who underwent cesarean section in our hospital from July 2022 to September 2023 were selected and randomly divided into a control group and an observation group using a random number table method.The control group received combined spinal epidural anesthesia,while the observation group received intravenous infusion of dexmedetomidine and combined spinal epidural anesthesia.The application effects,stress reactions,and hemodynamics were compared between the two groups.Results The Ramsay score of the observation group was higher than that of the control group at the time of incision suturing(T2)and 2 hours after surgery(T3),while the visual analogue score(VAS)was lower than that of the control group(P<0.05);the levels of cortisol(Cor),norepinephrine(NE),and C-reactive protein(CRP)in the observation group were lower than those in the control group at T2 and T3 stages(P<0.05);the average arterial pressure(MAP)and heart rate(HR)of the observation group were higher than those of the control group in stages T2 and T3(P<0.05).Conclusion Intravenous infusion of dexmedetomidine has a significant anesthetic effect in combined lumbar epidural anesthesia for cesarean section,can alleviate stress reactions,maintain hemodynamic stability,and is worth promoting.
