隔姜灸与腕踝针治疗脊髓损伤后尿潴留的临床研究

摘　　要：目的探讨脊髓损伤(SCI)后神经源性膀胱尿潴留患者应用隔姜灸联合腕踝针治疗的效果。方法选择2022年1月至2023年12月驻马店市中医院收治的106例SCI后神经源性膀胱尿潴留患者,采用随机数字表分为两组。对照组53例患者实施常规康复治疗,研究组53例患者实施常规康复治疗+隔姜灸联合腕踝针治疗,比较两组排尿日记、尿动力学指标、膀胱功能、生活质量[泌尿症状困扰评分(USDS)]、抑郁情绪[焦虑自评量表(SAS)]。结果研究组患者治疗后最大尿流率(13.12±2.23)mL/s、最大排尿量(241.37±52.13)mL、膀胱最大容量(433.82±48.63)mL,高于对照组的(10.65±2.17)mL/s、(184.26±50.44)mL、(405.12±50.04)mL,充盈期膀胱压、平均排尿次数、尿失禁次数、膀胱残余尿量、膀胱功能评分、USDS、SAS评分低于对照组(P<0.05)。结论SCI后神经源性膀胱尿潴留患者应用隔姜灸联合腕踝针治疗可有效促进排尿,减轻尿潴留,其作用机制可能与改善膀胱功能有关,有助于改善患者生活质量。Objective To investigate the effect of ginger separated moxibustion combined with wrist and ankle acupuncture on patients with neurogenic bladder urinary retention after spinal cord injury(SCI).Methods A total of 106 patients with neurogenic bladder urinary retention after SCI admitted to Zhumadian TCM Hospital from January 2022 to December 2023 were divided into control group(n=53)and study group(n=53)by random number table.All patients were managed by routine rehabilitation treatment,and those in study group were additionally treated with ginger separated moxibustion combined with wrist and ankle acupuncture.Urinary diaries,urodynamic indexes,bladder function,quality of life assessed by the urinary symptom distress score(USDS)and depression assessed by the self rating anxiety scale(SAS)were compared between the two groups.Results The maximum urine flow rate([13.12±2.23]mL/s VS[10.65±2.17]mL/s),the maximum urine volume([241.37±52.13]mL VS[184.26±50.44]mL),and the maximum bladder volume([433.82±48.63]mL VS[405.12±50.04]mL)in the study group were significantly higher than those of the control group(P<0.05).The bladder pressure,mean urination times,incontinence times,bladder residual urine volume,bladder function scores,USDS and SAS scores in the filling period were significantly lower in the study group than those of the control group(P<0.05).Conclusion Ginger separated moxibustion combined with wrist and ankle acupuncture can effectively promote urination and reduce urinary retention in patients with neurogenic bladder urinary retention after SCI,and improve the quality of life by improving bladder function.
