子午流注择时穴位贴敷联合加味温胆汤治疗肝胃不和型妊娠恶阻31例

摘　　要：目的 观察子午流注择时穴位贴敷联合加味温胆汤对肝胃不和型妊娠恶阻的疗效。方法 收集2021年1月~2023年11月在福建中医药大学附属第二人民医院产科住院的肝胃不和型妊娠恶阻62例进行回顾性分析,根据治疗方法不同分为单一补液组(A组)31例和在A组基础上进行子午流注择时穴位贴敷联合加味温胆汤组(B组)31例,对两组患者恶心呕吐(PUQE)评分、妊娠焦虑量表评分、尿酮体水平及转阴率、重返住院率等情况进行分析、总结。结果 治疗后,两组患者恶心呕吐(PUQE)评分、妊娠焦虑量表评分、尿酮体水平均低于治疗前(P<0.05);B组恶心呕吐(PUQE)评分、妊娠焦虑量表评分、尿酮体水平均低于A组且B组优于A组(P<0.05)。B组尿酮体转阴率54.84%,高于A组29.03%(P<0.05);B组重返住院率3.22%,低于A组22.58%(P<0.05)。结论 子午流注择时穴位贴敷联合加味温胆汤治疗能够改善患者恶心呕吐症状,减轻患者的妊娠焦虑状态,改善尿酮体水平,提高尿酮体转阴率,减少重返住院率,且操作简单方便,效果优于单一静脉补液。OBJECTIVE To observe the curative effect of acupoint application at proper times in accordance to meridian flow theory & modified warming-gallbladder prescription on pregnancy obstruction of liver-stomach disharmony type patients.METHODS Analysis was made on the 62 cases of liver-stomach disharmony pregnancy obstruction who were hospitalized at the Department of Obstetrics of the 2nd Affiliated People′s Hospital of Fujian University of Traditional Chinese Medicine between January 2021 and November 2023.31 cases were treated with rehydration(Group A) and another 31 cases were treated with acupoint application at proper times & modified warming-gallbladder prescription(Group B).Study was focused on nausea and vomiting(PUQE) score,pregnancy anxiety scale score,urinary ketone level and conversion to negative rate,and re-hospitalization rate of 2 groups of patients.RESULTS After treatment,the nausea and vomiting(PUQE) score,pregnancy anxiety scale score,urinary ketone level of all the patients were lower than before the treatment(P<0.05).The nausea and vomiting(PUQE) score,pregnancy anxiety scale score,urinary ketone level of group B were lower than group A(P<0.05).The conversion to negative rate of urine ketone in Group B was 54.84%,which was higher than Group A′s 29.03%(P<0.05);the re-hospitalization rate of Group B was 3.22%,which was lower than Group A′s 22.58%(P<0.05).CONCLUSION The treatment of acupoint application at due times together with modified warming-gallbladder prescription can improve the patient′s nausea and vomiting symptoms,reduce the patient′s pregnancy anxiety state,improve the urinary ketone level and its conversion to negative rate,reduce the rate of re-hospitalization.The operation is easy and convenient to take,and the effect is better than that of rehydration.
