摘　　要：目的:研究硕通镜下输尿管取石术(NPCU)和经皮肾镜碎石取石术(PCNL)治疗输尿管上段结石患者的效果。方法:回顾性分析2017年9月—2022年12月江门市新会区中医院67例输尿管上段结石患者的临床资料,根据不同手术方式分为NPCU组(n=44,采用NPCU)和PCNL组(n=23,采用PCNL)。比较两组患者手术指标、手术前后肾功能指标及围手术期并发症情况。结果:NPCU组手术时间长于PCNL组,术中出血量、住院时间、一次性结石清除率优于PCNL组,差异有统计学意义(P<0.05);术后3 d,NPCU组的血尿素氮(BUN)、血肌酐(Scr)水平低于PCNL组,差异有统计学意义(P<0.05);两组围手术期并发症总发生率比较,差异无统计学意义(P>0.05)。结论:NPCU和PCNL均是输尿管上段结石的有效治疗手段,相较于PCNL,NPCU的结石清除率更高,术中出血量少,且对肾功能影响小,术后恢复更快,但手术时间更长,可在患者符合手术条件时进行选择。Objective:To study the clinical efficacy of negative pressure combined urethroscope(NPCU)and percutaneous nephrolithotomy(PCNL)in patients with upper ureteral stones.Method:The clinical data of 67 patients with upper ureteral stones in Xinhui Hospital of Traditional Chinese Medicine of Jiangmen City from September 2017 to December 2022 were retrospectively analyzed.According to different surgical methods,they were divided into NPCU group(n=44,used NPCU)and PCNL group(n=23,used PCNL).The surgical indicators,preoperative and postoperative renal function indicators,and perioperative complications between two groups of patients were compared.Result:The surgery time in the NPCU group was longer than that in the PCNL group,intraoperative blood loss,hospital stay,one-time stone clearance rate in the NPCU group were better than those in the PCNL group,the differences were statistically significant(P<0.05).Three days after surgery,the levels of blood urea nitrogen(BUN)and creatinine(Scr)in the NPCU group were lower than those in the PCNL group,the differences were statistically significant(P<0.05).There was no statistically significant in total incidence of perioperative complications between the two groups(P>0.05).Conclusion:NPCU and PCNL are both effective treatment methods for upper ureteral stones.Compared to PCNL,NPCU has a higher stone clearance rate,lower intraoperative bleeding,less impact on renal function,faster postoperative recovery,but longer surgical time,which can be selected when the patient meets the surgical conditions.
