摘　　要：目的:本文主要以薛己《内科摘要》水肿医案为研究对象,总结吴门医家薛己论治水肿病的用药特色及辨治思想。其所载医案多从脏腑虚损出发,尤以脾胃肾为主。薛己临证善用补法,重视顾护人体正气,强调脾肾为本。治法上或朝夕互补,或以补为主,或补中兼泻;薛己临证擅化裁古方,多用补中益气汤、六君子汤、六味丸、八味丸类方等治疗水肿,或单用,或兼用,共奏利水消肿之功效。本文以《内科摘要》治疗水肿之法为指导,效用临床,疗效颇著,最后附一典型医案加以例证。Purpose:This article delves into the treatment of edema as depicted in Xue Ji's“Synopsis of Internal Medicine,”aiming to encapsulate the medicinal characteristics and treatment principles of Wu school physician Xue Ji in addressing edema.Xue Ji's clinical cases often stem from visceral deficiencies,with particular emphasis on the spleen,stomach,and kidneys.Xue Ji adeptly employs tonification methods,prioritizing the preservation of righteous qi and underscoring the pivotal role of the spleen and kidneys.Treatment strategies may intertwine complementary approaches,prioritize tonification,or integrate tonification with purgation.Proficient in adapting ancient formulas,Xue Ji frequently employs prescriptions such as Bu Zhong Yi Qi Tang,Liu Jun Zi Tang,Liu Wei Wan,and Ba Wei Wan to treat edema,either individually or in combination,to facilitate diuresis and reduce swelling.This article,guided by the treatment modalities outlined in the“Synopsis of Internal Medicine,”showcases notable clinical efficacy and concludes with a representative case study for elucidation.
