巴瑞替尼成功治疗坏疽性脓皮病1例

摘　　要：患者女,59岁,双小腿皮肤溃疡3年,复发加重3个月余。皮肤科情况:双下肢膝部下方至双足广泛色素沉着,肿胀明显,双小腿可见多处形状不规则溃疡,表面覆有黑色坏死组织,痂下有黄绿色脓液积聚,双小腿多处陈旧性瘢痕。综合病史、临床表现和组织病理,诊断符合:坏疽性脓皮病。患者接受巴瑞替尼治疗后,症状明显改善,双腿溃疡变浅,疼痛也明显减轻。目前随访5个月,未见坏疽性脓皮病复发。A 59-year-old female patient presented with skin ulcers on both legs last for 3 years that had recurred and worsened over more than 3 months.On dermatological examination,the lower extremities from below the knees to the feet were extensively erythematous and edematous.Several irregular ulcers were found on the lower legs,covered with black necrotic tissue.Yellow-green purulent exudate was noted under the eschar,and several old scars were seen on the lower legs.Based on the patient′s history,clinical presentation and histopathological findings,the diagnosis of pyoderma gangrenosum was confirmed.After baricitinib treatment,the patient′s symptoms improved,the depth of the leg ulcers gradually decreased,and the pain was significantly relieved.After 5 months′follow-up,no recurrence of pyoderma gangrenosum was observed.
