摘　　要：患者男,48岁,双手散发红斑、丘疹渐加重10 d。皮肤科情况:右手食指末端、大鱼际、左侧拇指根部见3处硬币大小鲜红色斑块,境界清楚,质地稍硬,表面覆以脓疱,中央轻度痂皮和凹陷,痂周可见灰白色晕,最外周可见红晕。皮损组织病理示:局部表皮上层细胞空泡化变性,胞浆内可见嗜酸性包涵体,局灶见水疱及微脓肿形成,结合病史提示“病毒感染”可能。皮肤镜示:外周黄白色环,中央可见多形性血管。诊断:羊痘。A 48-year-old male was admitted with progressively worsening sporadic erythematosus papules on both hands for 10 days.Dermatological examination showed that there were three coin-sized bright red plaques with a clear border and a slightly hard texture at the tip of the right index finger,the thenar area,and the proximal end of the left thumb,which was covered with pustules on the surface.The central scab was lightly crusted and sunken,and halos could be seen in greyish white around the scab and in red around the outermost.Histopathologic analysis of the skin biopsy specimen showed local vacuolar degeneration of the upper layer epidermal cells,eosinophilic inclusions in the cytoplasm,focal blisters and microabscess formation.Medical history suggested“viral infection”was possible.Dermoscopic examination showed peripheral yellow and white rings,central visible pleomorphic vessels.The diagnosis was Orf.
