百令胶囊在慢性阻塞性肺病急性加重期联合乙酰半胱氨酸溶液雾化吸入的临床疗效观察及对MPO、IL-2及IL-6水平的影响

摘　　要：目的探讨百令胶囊在慢性阻塞性肺病急性加重期(AECOPD)联合乙酰半胱氨酸溶液雾化吸入的临床疗效及对髓过氧化物酶(MPO)、人白细胞介素(IL)-2、IL-6水平的影响。方法收集在该院2020年9月至2022年9月慢性阻塞性肺病急性加期住院患者78例,随机分2组,对照组予以吸入用乙酰半胱氨酸溶液1.5 mL雾化吸入2 d,并进行常规治疗(包括抗炎、平喘、祛痰等)。研究组加用百令胶囊1.0 g口服3次/天,以对照组为基础。入院后进行《慢阻肺测试评估表(CAT评分)》,肺功能检测:第1秒用力呼出气容积占预计值百分比(FEV1/pred%)、第1秒用力呼出气容积/用力肺活量比值(FEV1/FVC%)、最大通气量占预计值百分比(MVV/pred%),对血液中MPO、IL-2、IL-6含量予以检测。治疗二周后再次复查上述指标,与治疗前后指标的变化进行对比,并进行统计学分析。结果CAT评分在干预后研究组平均值(11.16±2.79)低于对照组的结果(P<0.01)。研究组干预后肺功能:FEV1/pred%(72.83±3.96)%、FEV1/FVC%(67.14±2.58)%及MVV/pred%(55.36±4.93)%均高于对照组(64.48±4.25)%、(60.92±4.32)%及(41.72±5.31)%(P<0.05)。干预后研究组炎性因子:MPO(31.84±2.44)ng/mL、IL-2(0.65±1.07)pg/mL、IL-6(0.48±1.15)pg/mL,低于对照组(37.62±4.13)ng/mL、(1.12±0.94)pg/mL、(0.93±0.72)pg/mL(P<0.05)。结论百令胶囊联合乙酰半胱氨酸溶液雾化治疗在AECOPD患者中能减轻肺功能损害、降低炎性因子水平,具有积极的临床意义。Objective To investigate the clinical efficacy of Bailing Capsule combined with acetylcysteine solution nebulization inhalation in acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its effects on the levels of myeloperoxidase(MPO),human interleukin-2(IL),and IL-6.Methods A total of 78 hospitalized patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)from September 2020 to September 2022 were randomly divided into two groups.The control group received inhala-tion of 1.5ml of acetylcysteine solution via nebulization for 2 days and received routine treatment(including anti-inflammatory,anti asthmatic,expectorant,etc.).The research group added 1.0g of Bailing capsules orally 3/day,based on the control group.After admission,the Chronic Obstructive Pulmonary Disease(COPD)Test Evaluation Form(CAT Score)was performed,and lung function tests included the percentage of forced expir-atory volume in the first second to the expected value(FEV1/pre%),the ratio of forced expiratory volume in the first second to forced vital capacity(FEV1/FVC%),and the percentage of maximum ventilation to the ex-pected value(MVV/pre%).The levels of MPO,IL-2,and IL-6 in the blood were also measured.After two weeks of treatment,the above indicators were rechecked and compared with the changes in indicators before and after treatment,and statistical analysis was be conducted.Results The average CAT score in the inter-vention group was 11.16±2.79,which was lower than that in the control group(P<0.01).After interven-tion,the lung function in the research group showed that FEV1/pre%(72.83±3.96)%,FEV1/FVC%(67.14±2.58)%,and MVV/pre%(55.36±4.93)%were higher than(64.48±4.25)%,(60.92±4.32)%,and(41.72±5.31)%in the control group(P<0.05).After intervention,the inflammatory factors in the study group were:MPO(31.84±2.44)ng/mL,IL-2(0.65±1.07)pg/mL,IL-6(0.48±1.15)pg/mL,which were lower than(37.62±4.13)ng/mL,(1.12±0.94)pg/mL,and(0.93±0.72)pg/mL in the control group(P<0.05).Conclusion The combination of B
