佛山地区Rh血型分布调查及稀有血型库建立与应用

摘　　要：目的调查并分析佛山地区无偿献血者Rh血型分布情况及建立Rh稀有血型库。方法使用抗-D、抗-C、抗-c、抗-E和抗-e血清进行Rh血型分型,对RhD抗原筛查阴性的标本使用3批不同厂家IgG抗-D血清采用抗人球蛋白法进行确认。计算各表型百分率,Rh血型地区分布差异性采用χ2检验。通过血站信息系统建立稀有血型献血者详细的资料档案库。将RhD阴性的红细胞制备成含40%甘油的冰冻红细胞,建立献血者稀有血型实体库。结果佛山地区无偿献血者RhCE抗原分布趋势,按分布频率大小排列为CCee>CcEe>Ccee>ccEE>ccEe>CCEe>ccee>CcEE>CCEE,Rh系统4个主要抗原频率分布由高到低均为:e>C>c>E。本研究中各RhCE抗原在不同地区的分布情况差异有统计学意义(P<0.05)。对于佛山地区RhD阳性与阴性的无偿献血人群,其RhCE抗原分布频率差异较大,RhD阳性献血者以CCee(55.19%)、CcEe(28.05%)为主,RhD阴性献血者以Ccee(49.60%)、ccee(30.40%)为主,显示RhCE抗原表达与RhD抗原表达存在相关关系。建立献血者Rh稀有血型信息库数据25911条。Rh D阴性的冰冻红细胞库存量为20袋,40 U。根据临床需要,为患者精准提供相应的血液。结论佛山地区无偿献血人群Rh血型分布既有地域特点也有汉族人群的共性;本研究结果对指导佛山地区科学地进行无偿献血招募及合理地储存血液,提高血站为稀有血型受血者及时供血的能力,具有重要的意义。Objective To investigate and analyze the distribution of Rh blood types among voluntary blood donors in Foshan area and establish a rare Rh blood group bank.Methods Anti-D,anti-C,anti-c,anti-E and anti-e reagents were used to identify Rh blood group system and the RhD(-)were confirmed by the anti-human globulin method using 3 batches of IgG anti-D reagents from different manufacturers.The percentage of each phenotype was calculated,and the regional distribution difference of Rh blood group was tested by 2 test.The detailed data archives of rare blood group donors were established through the blood bank information system.RhD-negative red blood cells were prepared into frozen red blood cells containing 40%glycerol,and the rare blood group entity bank of blood donors was established.Results The distribution trend of RhCE antigen in voluntary blood donors in Foshan was CCee>CcEe>Ccee>ccEE>ccEe>CCEe>ccee>CcEE>CCEE.The frequency distribution of four major antigens in Rh system was:e>C>c>E.In this study,the distribution of RhCE antigens in different regions was different(P<0.05).For the RhD positive and negative blood donors in Foshan area,the distribution frequency of RhCE antigen was quite different.The RhD positive blood donors were mainly CCee(55.19%)and CcEe(28.05%),and the RhD negative blood donors were mainly Ccee(49.60%)and ccee(30.40%),which showed that there was a correlation between the expression of RhCE and RhD antigen.25911 pieces of Rh rare blood group information database of blood donors were es-tablished.The storage capacity of Rh D-frozen red blood cells was 20 bags(40 U).According to clinical needs,corresponding blood were accurately provided for patients.Conclusion The distribution of Rh blood group system from voluntary blood donors in Foshan area has both geographic characteristics and the general characteristics of Han population;The results have important significance to guide the scientific recruitment of voluntary blood donation and the reasonable storage of blood in Foshan area,and improve th
