摘 要:目的统计上海市奉贤地区RhD阳性人群Rh血型抗原及表型分布特征,分析Rh表型检测在输血治疗中的影响。方法对795例Rh献血者及321例住院患者通过盐水介质法或微柱凝胶法进行RhC、c、E、e抗原的检测,并回顾性分析住院患者的不规则抗体筛查阳性率及红细胞输注疗效。结果共检出RhD阳性表型8种,Rh表型分布频率最高的为CCDee(43.55%)和CcDEe(34.86%),其后依次CcDee>ccDEE>ccDEe>ccDee>CcDEE>CCDEe,未检出稀有表型CCDEE。上海市奉贤地区RhD阳性人群的Rh表型分布在不同ABO血型之间差异无统计学意义(P>0.05),Rh抗原阳性率由高到低分别为e(87.81%)>C(56.18%)>c(46.77%)>E(46.77%)。不同Rh表型输血相容频率由高到低分别为CcDEe(100.00)>CcDee(53.23)>CCDEe(43.82)>CCDee(43.55)>ccDEe(12.19)>CcDEE(7.89)>ccDEE(7.62)>ccDee(0.81)>CCDEE(0)。Rh表型配合性输注使得不规则抗体筛查阳性率由0.95%降至0.61%,差异具有统计学意义(χ^(2)=4.419,P=0.036<0.05)。结论上海市奉贤地区RhD阳性人群表型分布特征,与华东地区人群的分布特征相符合。Rh表型检测以及配合性输注能够减少不规则抗体的产生,改善输血疗效,减少不良反应,对临床输血工作有着重要意义。Objective To analyze the distribution characteristics of Rh blood group antigen and phenotype in RhD positive population in Fengxian district of Shanghai,and to analyze the influence of Rh phenotype detection in blood transfusion treatment.Methods RhC,c,E,e antigens were detected in 795 Rh donors and 321 inpatients by saline medium method or microcolumn gel method,and the positive rate of irregular antibody screening and the efficacy of red blood cell infusion in inpatients were retrospectively analyzed.Results A total of 8 RhD positive phenotypes were detected.CCDee(43.55%)and CcDEe(34.86%)had the highest distribution frequency of Rh,followed by CcDee,CcDee,ccDEE,ccDEe,ccDee,CcDEE,CCDEe,and no rare phenotype ccDee was detected.There was no significant difference in the distribution of Rh phenotype among different ABO blood groups(P>0.05),and the Rh antigen positive rate was e(87.81%)>C(56.18%)>c(46.77%)>E(46.77%)from high to low.The transfusion compatibility frequencies of different Rh phenotypes from high to low were CcDEe(100.00),CcDee(53.23),CCDEe(43.82),CCDee(43.55),ccDEe(12.19),CcDEE(7.89),ccDEE(7.62),ccDee(0.81),CCDEE(0).The positive rate of irregular antibody screening decreased from 0.95%to 0.61%with the cooperative infusion of Rh phenotype(χ^(2)=4.419,P=0.036<0.05).Conclusion The phenotypic distribution of RhD positive population in Fengxian district of Shanghai is consistent with that in East China.Rh phenotype detection and cooperative infusion can reduce the production of irregular antibodies,improve transfusion efficacy and reduce adverse reactions,which is of great significance for clinical transfusion work.
