应用调神通督针法结合升阳益胃汤从脾虚论治耳鸣

摘　　要：耳鸣作为临床常见病、多发病,西医对其发病机制目前尚不明确,且临床治疗上无特效药。随着社会的进步,脾虚病机在耳鸣的发病中日渐突出。脾胃为后天之本,脾胃功能虚弱,一方面,无源生化气血,耳窍失养,发为耳鸣;另一方面,人之气机升降失常,清阳不升,浊阴不降亦可致耳鸣。调神通督针法以脑-神-督为核心,旨在调神通督,升阳健脾,开窍止鸣。通过应用调神通督针法结合升阳益胃汤加减治疗脾虚型耳鸣医案一则,展示论治思路,以期为临床治疗耳鸣提供参考。Tinnitus is a common and frequently-occurring disease in clinic,yet its pathogenesis is still unrevealed in western medicine,and there are no specific drugs for its clinical treatment.With the progress of society,spleen deficiency is increasingly perceived as a principal pathogenesis in the development of tinnitus.It is argued that the spleen and stomach serve as an acquired foundation for human's wellbeing.When the functions of the two organs weaken,they fail to generate and transform qi and blood,resulting in malnutrition of the ears and even tinnitus on the one hand.On the other hand,the abnormal qi ascending and descending can also cause clear yang to fail to ascend and turbid yin to fail to descend,leading to tinnitus ultimately.Tiaoshen Tongdu acupuncture takes the brain,spirit and governor vessel as the core,aiming at regulating spirit and dredging the governor vessel,ascending yang qi and strengthening the spleen,and opening the ear orifices to terminate tinnitus.A case is attached to the study to demonstrate the effect of the application of Tiaoshen Tongdu acupuncture combined with modified Shengyang Yiwei Decoction in the treatment of tinnitus with the syndrome of spleen deficiency,elaborating on the syndrome diff erentiation and treatment of the disease,hoping to provide a reference for its clinical treatment.
