“榆皇生肌散”外敷治疗肛瘘术后创面愈合

摘　　要：目的探究“榆皇生肌散”外敷对肛瘘术后创面愈合的疗效以及对患者血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、转化生长因子β_(1)(transforming growth factorβ_(1),TGF-β_(1))、肿瘤坏死因子α(tumor necrosis factorα,TNF-α)水平的影响。方法选取2022年9月-2023年9月期间于南京市中医院住院治疗的肛瘘术后患者70例作为研究对象,采用随机数字表法分为对照组与观察组,各35例,对照组术后进行常规护理,并给予凡士林纱条换药,观察组在对照组基础上给予“榆皇生肌散”外敷。比较2组创面愈合效果、干预前后血清VEGF、TGF-β_(1)、TNF-α水平差异。结果观察组治疗14 d时总有效率为94.29%,高于对照组的74.29%(P<0.05);观察组创面愈合时间短于对照组(P<0.05);治疗14 d后2组创面渗出、肉芽生长以及VAS评分相较治疗前均下降,且治疗后观察组以上评分较对照组更低,差异具有统计学意义(P<0.05);2组治疗14 d血清VEGF、TGF-β1水平较治疗前上升,TNF-α则下降,且观察组VEGF、TGF-β1、TNF-α水平变化幅度较对照组更大(P<0.05)。结论榆皇生肌散外敷可以有效提升肛瘘患者术后创面愈合的疗效,可以降低炎症水平、促进血管生长,有利于促进创面愈合。Objective To explore the eff ect of external application of Yuhuang Shengji Powder on wound healing after anal fistula operation and the influence on the serum levels of vascular endothelial growth factor(VEGF),transforming growth factorβ1(TGF-β1)and tumor necrosis factor-α(TNF-α)in patients.Methods Seventy patients with anal fistula who were hospitalized in the Nanjing Hospital of Chinese Medicine from September 2022 to September 2023 were selected as the study subjects,and were divided into two groups by random number table method,with 35 cases in each group.Group A received a routine postoperative care and was given vaseline gauze dressing change,while Group B was given external application of Yuhuang Shengji Powder on the basis of Group A.The effect of wound healing and the levels of serum VEGF,TGF-β1 and TNF-αwere compared before and after intervention between the two groups.Results The total effective rate of Group B was higher than Group A after 14 days of treatment(94.29%vs 74.29%,P<0.05).The wound healing time of Group B was significantly shorter than that of Group A(P<0.05).Compared with before treatment,the wound exudation,granulation growth and VAS scores of both groups decreased after 14 days of treatment,and the above scores of Group B were significantly lower compared with those in Group A(P<0.05).After 14 days of treatment,the serum levels of VEGF and TGF-β1 in the two groups were increased,while TNF-αwas decreased,and the changes of VEGF,TGF-β1 and TNF-αin Group B were greater than those in Group A(P<0.05).Conclusion External application of Yuhuang Shengji Powder can effectively improve the curative effect of postoperative wound healing in patients with anal fistula,reduce the level of inflammation,promote the growth of blood vessels,and promote wound healing.
