丘墟透照海穴针刺法联合康复训练对脑梗死后偏瘫下肢运动功能的疗效观察

摘　　要：目的探讨丘墟透照海穴针刺法联合康复训练对脑梗死后偏瘫下肢运动功能的临床疗效。方法将82例脑梗死后偏瘫患者按照随机数字分组法分成观察组与对照组。对照组给予康复功能训练,观察组在康复训练基础上联合丘墟透照海穴针刺治疗。观察2组治疗前后脑卒中神经缺损程度量表(NIHSS)评分、Barthel指数(BI)、下肢Fugl-Meyer评定量表(FMA)评分、血清神经营养因子[神经生长因子(NGF)、脑源性神经营养因子(BDNF)]、下肢表面肌电图(sEMG)、运动学参数指标,统计2组总有效率,并评估2组治疗安全性。结果观察组治疗后NIHSS评分低于对照组(P<0.05),BI评分,FMA评分,血清NGF和BDNF水平,患侧下肢股直肌、股二头肌、胫骨前肌和腓肠肌的iEMG和RMS指标,运动学参数指标及治疗总有效率均高于对照组(P<0.05);2组治疗期间均未发生不良事件。结论丘墟透照海穴针刺法联合康复训练能够调节脑梗死后偏瘫患者血清神经营养因子,改善患肢表面肌电信号,有助于提高患者下肢运动功能,疗效显著。Objective To explore the clinical efficacy of Qiuxu Touzhao Haixu acupuncture combined with rehabilitation training on the motor function of hemiplegic lower limbs after cerebral infarction.Methods A total of 82 patients with hemiplegia after cerebral infarction were randomly divided into an observation group and a control group.The control group received rehabilitation functional training,while the observation group received combined treatment with Qiuxu Touzhao Haixu acupuncture on the basis of rehabilitation training.The scores of NIHSS,BI,FMA,the serum neurotrophic factors(NGF,BDNF),the lower limb surface electromyography(sEMG),the kinematic parameter indicators were observed in the two groups before and after treatment,and the total eff ective rates of the two groups were calculated,and the treatment safety was evaluated in both groups.Results The NIHSS scores of the observation group were lower than those of the control group after treatment(P<0.05),while the scores of BI,FMA,the levels of serum NGF and BDNF,the indicators of iEMG and RMS indicators,the affected lower limb rectus femoris,biceps femoris,anterior tibialis,and gastrocnemius muscles,the kinematic parameter indicators,as well as the total effective rate of treatment,were all higher than those of the control group(P<0.05).No adverse events occurred during the treatment in both groups.Conclusion The combination of Qiuxu Touzhao Haixu acupuncture and rehabilitation training can regulate serum neurotrophic factors in hemiplegic patients after cerebral infarction and improve surface electromyographic signals of affected limb,which help improve lower limb motor function,with significant therapeutic effects.
