急性脑梗死伴有2型糖尿病应用复元醒脑汤的临床干预研究

摘　　要：目的分析急性脑梗死伴有2型糖尿病应用复元醒脑汤的临床干预效果。方法选取2023年3月—2024年7月在北京市第一中西医结合医院治疗的120例急性脑梗死伴有2型糖尿病患者为研究对象,根据治疗方法的不同分为对照组(60例,常规治疗)、观察组(60例,常规治疗联合复原醒脑汤治疗)。对比两组血糖及胰岛素指标、神经功能、凝血功能、血管内皮功能相关指标及中医证候积分,并观察不良反应发生情况。结果治疗后,观察组空腹血糖、餐后2 h血糖、空腹胰岛素水平低于对照组,胰岛素敏感指数高于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组美国国立卫生研究院卒中量表评分、D-二聚体、内皮素-1水平低于对照组,一氧化氮水平高于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组中医证候积分低于对照组,差异均有统计学意义(P均<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论急性脑梗死伴有2型糖尿病应用复元醒脑汤的临床干预效果显著,能调节血糖,缓解临床症状,对神经功能、凝血功能、血管内皮功能也能起到良好的改善作用,并且不会增加不良反应。Objective To analyze the clinical intervention effect of Fuyuan Xingnao Decoction on acute cerebral infarction with type 2 diabetes mellitus.Methods A total of 120 patients with acute cerebral infarction and type 2 diabetes mellitus treated in Beijing First Integrated Traditional Chinese and Western Medicine Hospital from March 2023 to July 2024 were selected as the research objects.According to the different treatment methods,they were divided into control group(60 cases,treated with conventional methods)and observation group(60 cases,treated with conventional methods combined with Fuyuan Xingnao Decoction).The related indicators of blood glucose and insulin,neurological function,coagulation function,vascular endothelial function and traditional Chinese medicine syndrome scores were compared between the two groups,and the occurrence of adverse reactions was observed.Results After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose and fasting insulin in the observation group were lower than those in the control group,and the insulin sensitivity index was higher than that in the control group,the differences were statistically significant(all P<0.05).After treatment,the National Institute of Health Stroke Scale score,D-dimer and endothelin-1 levels in the observation group were lower than those in the control group,and nitrogen oxide level was higher than that in the control group,the differences were statistically significant(all P<0.05).After treatment,the traditional Chinese medicine syndrome scores of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The clinical intervention effect of Fuyuan Xingnao Decoction on acute cerebral infarction with type 2 diabetes mellitus is significant.It can regulate blood glucose,relieve clinical symptoms,improve neurological function,coagulation function a
