镍钛形状记忆合金环抱器内固定与胸部护板外固定在多发肋骨骨折治疗中的疗效比较研究

摘　　要：目的对比镍钛形状记忆合金环抱器内固定与胸部护板外固定在多发肋骨骨折治疗中的效果。方法随机选取2021年1月—2024年2月在青岛市即墨区人民医院接受治疗的80例多发肋骨骨折患者为研究对象。根据不同治疗方法分为对比组(40例)和探究组(40例)。对比组采用胸部护板外固定治疗,探究组则采用镍钛形状记忆合金环抱器内固定治疗。对比两组的手术指标、血气指标、疼痛介质和并发症总发生率。结果探究组下床活动时间、住院时间和骨折愈合时间均优于对比组,差异均有统计学意义(P均<0.05)。术后1 d,探究组血气指标优于对比组,差异有统计学意义(P<0.05)。术后1 d,探究组血清疼痛介质水平低于对比组,差异有统计学意义(P<0.05)。探究组并发症总发生率为5.00%(2/40),低于对比组的20.00%(8/40),差异有统计学意义(χ^(2)=4.114,P<0.05)。结论镍钛形状记忆合金环抱器内固定与胸部护板外固定相比,可缩短下床活动时间、住院时间和骨折愈合时间,同时能改善血气指标,降低血清疼痛介质水平,并减少并发症的发生。Objective To compare the effect of nickel-titanium shape memory alloy embracing instrument internal fixation and chest shield external fixation in the treatment of multiple rib fractures.Methods A total of 80 patients with multiple rib fractures who were treated in The People's Hospital of Jimo.Qingdao from January 2021 to February 2024 were randomly selected as the study subjects.According to different treatment methods,they were divided into comparison group(40 cases)and inquiry group(40 cases).The control group was treated with external fixation of chest guard plate,while the study group was treated with internal fixation of nickel-titanium shape memory alloy embracing instrument.The surgical indexes,blood gas indexes,pain mediators and total incidence of complications were com-pared between the two groups.Results The time of getting out of bed,hospitalization time and fracture healing time in the inquiry group were better than those in the comparison group,and the differences were statistically significant(all P<0.05).At 1 day after operation,the blood gas index of the inquiry group was better than that of the comparison group,and the difference was statistically significant(P<0.05).1 day after operation,the level of serum pain mediators in the inquiry group was lower than that in the comparison group,and the difference was statistically significant(P<0.05).The total incidence of complications in the inquiry group was 5.00%(2/40),which was lower than 20.00%(8/40)in the comparison group,and the difference was statistically significant(χ^(2)=4.114,P<0.05).Conclusion Com-pared with chest guard plate external fixation,nickel-titanium shape memory alloy embracing instrument internal fixa-tion can shorten the time of getting out of bed,hospitalization time and fracture healing time,improve blood gas index,reduce serum pain medium level and reduce complications.
