健脾益肾增液汤在老年慢性功能性便秘患者中的应用及对肠道神经递质的影响

摘　　要：目的 研讨健脾益肾增液汤对老年慢性功能性便秘(chronic functional constipation,CFC)患者肠道神经递质的影响及应用效果分析及对肠道神经递质的影响。方法 随机选取2021年5月—2024年1月厦门市同安区中医医院确诊的70例CFC患者为研究对象,根据不同治疗方法分为对照组和观察组,每组35例。对照组予以常规对症处理,观察组在对照组的基础上温服健脾益肾增液汤。比较两组的疗效、中医证候评估情况、排便状况以及肠道神经递质水平。结果 观察组的治疗总有效率为94.29%(33/35)高于对照组的77.14%(27/35),差异有统计学意义(χ^(2)=4.200,P=0.040)。治疗后,观察组中医证候(汗出气短、便难排出、便后乏力及肢倦懒言)评分均低于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组排便时间及频次均少于对照组,差异均有统计学意义(P均<0.05)。治疗后,观察组肠道神经递质水平低于对照组,差异有统计学意义(P<0.05)。结论 健脾益肾增液汤能够提升老年CFC患者的治疗效果,有效改善患者的便秘症状以及排便状况,调节肠道神经递质。Objective To investigate the application of Jianpi Yishen Zengye Decoction in elderly patients with chronic functional constipation(CFC) and its effect on intestinal neurotransmitters.Methods A total of 70 patients with CFC diagnosed in Xiamen Tong'an District Hospital of Traditional Chinese Medicine from May 2021 to January 2024 were randomly selected as the research objects.According to different treatment methods,they were divided into control group and observation group,with 35 cases in each group.The control group was given routine symptomatic treatment,and the observation group was given Jianpi Yishen Zengye decoction on the basis of the control group.The efficacy,traditional Chinese medicine syndrome evaluation,defecation status and intestinal neurotransmitter levels were compared between the two groups.Results The total effective rate of treatment in the observation group was 94.29%(33/35),which was higher than 77.14%(27/35) in the control group,and the difference was statistically significant(χ^(2)=4.200,P=0.040).After treatment,the scores of traditional Chinese medicine syndromes(shortness of breath after sweating,difficulty in defecation,fatigue after defecation and tiredness of limbs) in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the defecation time and frequency of the observation group were less than those of the control group,and the differences were statistically significant(both P<0.05).After treatment,the level of intestinal neurotransmitters in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Jianpi Yishen Zengye Decoction can improve the therapeutic effect of elderly patients with CFC,effectively improve the constipation symptoms and defecation status of patients,and regulate intestinal neurotransmitters.
