摘　　要：目的探讨抑郁症患者自主神经功能损伤与微伏级T波电交替(MTWA)的相关性。方法选取2022年1月至2023年10月我院收治的符合抑郁症诊断的患者50例为研究对象,检测心率变异性(HRV)、心率减速力(DC)、心率加速力(AC)、连续心率减速力(DRs)评估自主神经功能。根据MTWA的发生分为MTWA阳性组和MTWA阴性组,比较两组患者的自主神经功能损伤程度及其可能的影响因素。采用Spearman分析自主神经功能损伤与MTWA的相关性。结果MTWA阳性组和MTWA阴性组全部NN间期的标准差(SDNN)、总频谱(TP)、高频带(HF)、DC、AC、DR4、血钾比较,差异有统计学意义(P<0.05),Logistic回归分析显示:SDNN数值降低是抑郁症患者发生MTWA的危险因素。Spearman相关性分析显示:AC与MTWA呈正相关,DC、SDNN、血钾与MTWA呈负相关(P<0.05)。结论抑郁症患者自主神经功能损伤与MTWA的发生存在一定的相关性,MTWA可预测恶性心律失常及心源性猝死的发生。Objective To explore the correlation between autonomic nervous system dysfunction and microvolt level T-wave alternation(MTWA)in patients with depression.Methods Fifty patients diagnosed with depression admitted to our hospital from January 2022 to October 2023 were selected as the research subjects.Heart rate variability(HRV),heart rate deceleration(DC),heart rate acceleration(AC),and heart rate deceleration runs(DRs)were measured to evaluate autonomic nervous system function.According to the occurrence of MTWA,they were divided into MTWA positive group and MTWA negative group.The degree of autonomic nervous system damage and possible influencing factors in the two groups of patients were compared.Use Spearman analysis to investigate the correlation between autonomic nervous system dysfunction and MTWA.Results The difference in the standard deviation of the NN(SDNN),total power(TP),high-frequency power(HF),DC,AC,DR4,and blood potassium between the MTWA positive group and the MTWA negative group was statistically significant(P<0.05).Logistic regression analysis showed that a decrease in SDNN values is a risk factor for MTWA in patients with depression.Spearman correlation analysis showed that AC was positively correlated with MTWA,while DC,SDNN,blood potassium were negatively correlated with MTWA(P<0.05).Conclusions There is a certain correlation between autonomic dysfunction in patients with depression and the occurrence of MTWA,which can serve as a risk indicator for predicting malignant arrhythmias and sudden cardiac death events.
