原发不孕和继发不孕患者生殖道病原体的聚合酶链反应及导流杂交技术检测情况分析

摘  要:目的探讨原发不孕和继发不孕患者的脲原体(UU及Uup1、Uup3、Uup6、Uup14)、人型支原体(MH)、生殖支原体(MG)、沙眼衣原体(CT)、淋病奈瑟菌(NG)及单纯疱疹病毒Ⅱ型(HSV-Ⅱ型)的生殖道感染状况。方法选取2021年2月—2022年9月清远市妇幼保健院诊治的300例原发不孕患者和300例继发不患者为研究对象,运用聚合酶链反应(PCR)及导流杂交法技术检测宫颈分泌物的10项生殖道病原体感染情况,并比较这些病原体在原发不孕组和继发不孕组的组间差异。结果继发不孕组和原发不孕组总感染率分别为92.67%和53.67%,其中不孕患者最常见的病原体感染为微小脲原体Uup6型和Uup3型、解脲脲原体UU、沙眼衣原体CT及人型支原体MH。继发不孕组和原发不孕组感染微小脲原体Uup6感染率分别为32.33%和17.00%,微小脲原体Uup3感染率分别为26.67%和17.67%,其次是解脲脲原体UU(16.33%和8.33%)、沙眼衣原体CT(14.67%和8.00%)及人型支原体MH(15.33%和6.00%)。而单纯疱疹病毒Ⅱ型(2.33%和0.67%)、淋病奈瑟菌NG(2%和0.00%)及生殖支原体MG(1.33%和0.67%)感染率较低。与原发不孕组相比,继发不孕组的微小脲原体Uup6、Uup3和解脲脲原体UU、沙眼衣原体CT及人型支原体MH比原发不孕组感染率显著升高,单纯疱疹病毒Ⅱ型和生殖支原体MG的感染率在两组间差异无统计学意义(P<0.05)。继发不孕组2-4项的混合感染率(23.67%)显著高于原发不孕组(9.00%)。结论继发不孕比原发不孕发生生殖道感染率更高,UP、UU及CT是最常见的病原体,需要重视UP的检测,使用PCR及导流杂交技术用于同时检测多项病原体可减少漏诊。Objective To investigate the genital tract pathogens infection status of Ureaplasma Urealyticum(UU and four subtypes of Uup1,Uup3,Uup6 and Uup14),Mycoplasma hominis(MH),Mycoplasma genitalun(MG),Chlamydia trachomatis(CT),Neisseria gonorrhoeae(NG)and Heipes simplex virus typeⅡ(HSV-Ⅱ)in patients with primary and secondary infertility.Methods From February 2021 to September 2022,300 primary infertility patients and 300 secondary infertility patients were included in this study from Maternal and Child Health Hospital of Qingyuan city.PCR flow-through hybridization were used to detected these ten genital tract pathogens in cervical secretions,and the differences of these pathogens between the primary infertility group and the secondary infertility group were compared.Results The total infection rates of secondary infertility group and primary infertility group were 92.67%and 53.67%,respectively.The most common pathogens in infertility patients were Uup6 and Uup3,UU,CT and MH.The infection rates of Uup6 and Uup3 in the secondary infertility group vs primary infertility group were 32.33%vs.17.00%and 26.67%vs.17.67%,respectively,followed by UU(16.33%vs.8.33%),CT(14.67%vs.8.00%)and MH(15.33%vs.6.00%).The infection rates of HSV-Ⅱ(2.33%vs.0.67%),NG(2.00%vs.0.00%)and MG(1.33%vs.0.67%)were lower.Compared with the primary infertility group,the infection rates of Uup6,Uup3,UU,CT and MH in the secondary infertility group were significantly higher.The infection rates of HSV-Ⅱand MG had no significant difference between the two groups.The mixed infection rate of 2-4 items in the secondary infertility group(23.67%)was significantly higher than that in the primary infertility group(9.00%).Conclusion The infection rate of genital tract pathogens in secondary infertility is higher than that in primary infertility.UP,UU and CT are the most common pathogens,which inspire us to pay more attention to the detection of UP.PCR flow-through hybridization technology for detecting multiple pathogens simultaneously can reduce missed diagn
