无张力尿道中段悬吊术对女性混合性尿失禁患者性生活质量满意度及不良反应的影响

摘　　要：目的探讨无张力尿道中段悬吊术对女性混合性尿失禁患者性生活质量、满意度及不良反应的影响,为临床治疗混合性尿失禁提供参考。方法选取2018年10月—2020年10月在金华市中心医院行无张力尿道中段悬吊术的198例女性混合性尿失禁患者。分析患者的一般资料、术前和术后盆腔脏器脱垂/尿失禁性功能问卷(PISQ-12)评分、满意度评分以及不良反应发生率。结果27例患者出现术后不良反应,性交疼痛、股内侧疼痛、神经损伤、排尿困难、泌尿系统感染的例数分别为3例、6例、9例、6例、3例。不同手术方式患者的满意度评分比较,差异均无统计学意义(均P>0.05)。患者术前和术后PISQ-12问卷总评分分别为(30.65±6.52)分和(32.16±5.67)分,生理因素评分分别为(14.69±3.67)分和(17.67±2.67)分,性伴侣因素评分分别为(7.08±1.82)分和(7.21±71.37)分,情感因素评分分别为(7.86±3.06)分和(7.76±3.21)分。结论无张力尿道中段悬吊术能提升女性混合性尿失禁患者的性生活质量,提高患者的满意度,减少不良反应发生率。Objective To explore the effect of tension-free middle urethral suspension on sexual life quality,satisfaction,and adverse reactions in female patients with mixed urinary incontinence,provide reference for clinical treatment of mixed urinary incontinence.Methods A total of 198 female patients with mixed urinary incontinence who underwent tension-free middle urethral suspension in Jinhua Municipal Central Hospita from October 2018 to October 2020 were selected.The general data,scores of preoperative and postoperative pelvic organ prolapse/urinary incontinence sexual questionnaire(PISQ-12)and satisfaction,incidence rate of adverse reactions were analyzed.Results There were 27 patients with postoperative adverse reactions,the numbers of patients with sexual intercourse pain,medial femoral pain,nerve injury,dysuria,and urinary system infection were 3,6,9,6,and 3 cases,respectively.There was no significant difference in satisfaction score between two kinds of surgical methods(P>0.05).The total scores of PISQ-12 questionnaire before and after operation were(30.65±6.52)and(32.16±5.67),respectively,physiological scores were(14.69±3.67)and(17.67±2.67),respectively,sexual partner scores were(7.08±1.82)and(7.21±71.37),respectively,emotional factor scores were(7.86±3.06)and(7.76±3.21),respectively.Conclusion Tension-free middle urethral suspension can improve the quality of sexual life of female patients with mixed urinary incontinence,improve satisfaction,and reduce the incidence rate of adverse reactions.
