摘　　要：目的研究子宫输卵管动态三维超声造影(3D HyCoSy)评估不孕症患者输卵管通畅度的价值。方法研究对象为2019年1月—2021年12月在浙江省舟山医院就诊的84例不孕症妇女,回顾性分析3D HyCoSy检查结果,以腹腔镜检查结果为对照,绘制受试者工作特征(ROC)曲线,分析3D HyCoSy对输卵管通畅度的诊断价值。结果3D HyCoSy重建显示输卵管160条,8例既往有单侧输卵管切除手术史,其中输卵管通畅50条,通而不畅43条,阻塞67条。ROC曲线显示曲线下面积(AUC)为0.952,准确度、灵敏度、特异度分别为90.68%、93.81%、91.49%。84例不孕症患者3D HyCoSy检查后未见阴道感染发生,疼痛、阴道出血发生率分别为90.48%、75.00%,均为Ⅰ~Ⅱ度。结论3D HyCoSy诊断不孕症患者输卵管通畅度具有优势,虽有一定比例的疼痛、阴道出血发生,但未影响患者对检查的接受度及依从度。Objective To study the value of dynamic three-dimensional hysterosalpingo contrast sonography(3D HyCoSy)in evaluation of patency of fallopian tubes in infertile patients.Methods A total of 84 infertile women who were treated in Zhoushan Hospital from January 2019 to December 2021 were selected as study object,3D HyCoSy examination results were retrospectively analyzed,and the results of laparoscopy were used as control group to draw receiver operating characteristics(ROC)curve to analyze the diagnostic value of 3D HyCo-Sy for patency of fallopian tubes.Results 3D HyCoSy reconstruction showed 160 fallopian tubes,and 8 cases had history of unilateral salpingectomy;50 fallopian tubes were unobstructed,43 fallopian tubes were not unobstructed,and 67 cases were blocked.ROC curve analysis showed that the area under the curve(AUC)was 0.952.The accuracy,sensitivity,and specificity were 90.68%,93.81%,and 91.49%,respectively.After 3D HyCoSy examination of 84 infertile patients,there was no vaginal infection.The incidence rates of pain and vaginal bleeding were 90.48%and 75.00%,respectively,but they were all gradeⅠ-Ⅱ.Conclusion 3D HyCoSy has superior performance in diagnosing tubal patency in infertile patients.Although a certain proportion of pain and vaginal bleeding events occur,it does not affect acceptance and compliance with the examination of the patients.
