摘　　要：目的:观察右美托咪定联合丙泊酚对经尿道前列腺电切术(TURP)患者应激反应、生殖激素和血清神经生长因子(NGF)、神经元特异性烯醇化酶(NSE)、S100β蛋白的影响。方法:按照随机数字表法将160例TURP患者分为对照组(80例,丙泊酚麻醉)和观察组(80例,右美托咪定联合丙泊酚麻醉)。对比两组围术期指标、血清应激反应指标、生殖激素、术后谵妄发生率、血清NGF、NSE、S100β蛋白和不良反应。结果:与对照组相比,观察组的首次下床时间、苏醒时间、住院时间、首次进食时间均更短,术后谵妄发生率更低(P<0.05)。与对照组术后1 d相比,观察组的肾上腺素(NE)、皮质醇(Cor)、睾酮(T)、雌激素(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、NSE、S100β蛋白更低,NGF更高(P<0.05)。不良反应发生率:对照组6.25%与观察组5.00%对比无差异(P>0.05)。结论:右美托咪定联合丙泊酚可减轻TURP患者应激反应,降低术后谵妄发生率,改善生殖激素水平并调节血清NGF、NSE、S100β蛋白水平,且安全性好。Objective:To observe the effects of dexmedetomidine combined with propofol on stress response,reproductive hormones,serum nerve growth factor(NGF),neuron-specific enolase(NSE)and S100βprotein in patients undergoing transurethral resection of prostate(TURP).Methods:160 TURP patients were divided into control group(80 cases,propofol anesthesia)and observation group(80 cases,dexmedetomidine combined with propofol anesthesia)according to random number table method.The perioperative indexes,serum stress response indexes,reproductive hormones,incidence of postoperative delirium,serum NGF,NSE,S100βprotein and adverse reactions were compared between two groups.Results:Compared with control group,first time out of bed,recovery time,hospitalization time and first feeding time in observation group were shorter,and the incidence of postoperative delirium was lower(P<0.05).Compared with control group 1 d after operation,epinephrine(NE),cortisol(Cor),testosterone(T),estrogen(E2),follicle stimulating hormone(FSH),luteinizing hormone(LH),NSE,S100βprotein in observation group were lower,and NGF was higher(P<0.05).The incidence of adverse reactions:There was no difference between the control group of 6.25%and the observation group of 5.00%(P>0.05).Conclusion:The combination of dexmedetomidine and propofol can reduce the stress response,reduce the level of the incidence of postoperative delirium,improve reproductive hormones levels and regulate the levels of serum NGF,NSE and S100βprotein in TURP patients,and with good safety.
