金黄膏外敷联合功率自行车训练治疗膝骨关节炎的临床研究

摘　　要：[目的]观察金黄膏外敷联合功率自行车训练治疗膝骨关节炎(KOA)的临床疗效。[方法]研究纳入2019年1月—2022年6月在南京中医药大学附属苏州市中医医院骨伤科门诊确诊的KOA患者74例。通过随机数字表法,将患者分为治疗组和对照组,每组37例。治疗组使用金黄膏外敷联合功率自行车训练治疗,对照组单纯使用功率自行车训练治疗,两组疗程均为4周。比较两组治疗前、治疗后2周及治疗后4周膝部疼痛视觉模拟评分法(VAS)评分、膝关节周径、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分及关节腔积液厚度。[结果]对照组脱落2例,共完成病例72例,即对照组共35例,治疗组共37例。治疗前后组内比较,两组患膝疼痛VAS评分、膝关节周径、WOMAC评分及关节腔积液厚度差异均有统计学意义(P<0.05);组间治疗前后患膝疼痛VAS评分、膝关节周径、WOMAC评分及关节腔积液厚度比较,差异均有统计学意义(P<0.05)。[结论]金黄膏外敷联合功率自行车训练不仅能有效缓解KOA患者的疼痛,还能促进关节腔积液的吸收,进一步改善膝关节的功能。[Objective]To observe the clinical effect of golden paste combined with power bicycle training on knee osteoarthritis(KOA).[Methods]This study included 74 patients diagnosed with KOA in the Orthopedics and Traumatology Department of Suzhou Hospital of Traditional Chinese Medicine Affiliated to Nanjing University of Chinese Medicine from January 2019 to June 2022.The patients were divided into the treatment group and the control group by random number table method,37 cases in each group.The treatment group was treated with golden paste combined with power bicycle training,while the control group was treated with power bicycle training alone.The course of treatment in both groups was 4 weeks.Visual analogue scale(VAS)scores,knee circumference diameter,The Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores,and joint effusion thickness were compared between the two groups before treatment,2 weeks after treatment,and 4 weeks after treatment.[Results]A total of 72 cases were completed,including 35 cases in the control group,2 cases shedding,and 37 cases in the treatment group.Before and after treatment,there were statistically significant differences in VAS scores,knee circumference,WOMAC scores,and joint effusion thickness between the two groups(P<0.05).There were significant differences in VAS score,knee diameter,WOMAC score and joint effusion thickness between groups before and after treatment(P<0.05).[Conclusion]Golden paste combined with power bicycle training can not only effectively relieve the pain of KOA patients,but also promote the absorption of joint effusion,and further improve the function of knee joint.
