摘 要:目的探讨肺表面活性物质微创给药技术(LISA)注入肺泡表面活性物质(PS)联合经鼻持续气道正压通气(CPAP)治疗新生儿呼吸窘迫综合征(NRDS)的临床效果。方法选取2022年10月至2023年9月威海市妇幼保健院收治的96例NRDS患儿,以随机数字表法分为研究组(n=48)与对照组(n=48)。两组均给予经鼻CPAP,对照组给予气管插管-肺表面活性物质-拔管,研究组采用LISA注入PS,比较两组临床指标、血气指标、不良事件发生情况。结果研究组置管时间、机械通气时间、总用氧时间、住院时间较对照组更短(P<0.05);两组治疗3 h后血二氧化碳分压(PCO_(2))下降,血氧分压(PO_(2))升高,且研究组PCO_(2)较对照组更低(P<0.05),PO_(2)较对照组更高(P<0.05);研究组PS反流、气漏综合征、喉头损伤发生率较对照组更低(P<0.05);两组支气管肺发育不良、肺炎、肺出血发生率比较,差异无统计学意义(P>0.05)。结论LISA注入PS联合经鼻CPAP治疗NRDS的临床效果较好,可缩短置管、机械通气、住院时间,改善血气指标,减少不良事件发生。Objective To explore the clinical effect of less invasive surfactant administration(LISA)injecting pulmonary surfactant(PS)combined with nasal continuous positive airway pressure(CPAP)in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 96 children with NRDS who were admitted to Weihai Maternal and Child Health Care Hospital from October 2022 to September 2023 were selected and divided into study group(n=48)and control group(n=48)by the random number table method.Both groups received nasal CPAP,and the control group received tracheal intubation-pulmonary surfactant-extubation,the study group received LISA injecting PS.The clinical indicators,blood gas indicators,and adverse events in both groups were compared.Results The catheterization time,mechanical ventilation time,total oxygen consumption time,and hospitalization time in the study group were shorter than those in the control group(P<0.05).After 3 hours of treatment,the blood partial pressure of carbon dioxide(PCO_(2))decreased and the blood partial pressure of oxygen(PO_(2))increased,and the PCO_(2) of the study group was lower than that of the control group(P<0.05),and the PO_(2) was higher than that of the control group(P<0.05).The incidence of PS reflux,air leakage syndrome and laryngeal injury in the study group was lower than that in the control group(P<0.05).There was no significant difference in the incidence of bronchopulmonary dysplasia,pneumonia and pulmonary hemorrhage between the two groups(P>0.05).Conclusion The clinical effect of LISA injecting PS combined with nasal CPAP in the treatment of NRDS is good,which can shorten catheterization,mechanical ventilation,and hospitalization time,improve blood gas indicators,and red uce the incidence of adverse events.
