摘　　要：目的探讨瑞芬太尼在全身麻醉下腹腔镜子宫肌瘤剔除术中的应用效果。方法选取2021年3月至2023年6月于寿光市人民医院接受全身麻醉下腹腔镜子宫肌瘤剔除术的80例患者作为研究对象,以随机数字表法分为两组,每组各40例。对照组使用环泊酚和七氟醚进行麻醉,观察组在对照组基础上静脉持续泵注瑞芬太尼进行麻醉。比较两组术中环泊酚和七氟醚用量、平均动脉压及褪黑素水平。结果观察组术中环泊酚和七氟醚用量均少于对照组(P<0.05)。观察组麻醉时平均动脉压低于对照组,麻醉后10min、麻醉复苏时平均动脉压高于对照组(P<0.05)。麻醉苏醒时,观察组褪黑素水平高于对照组(P<0.05)。结论瑞芬太尼应用于全身麻醉下腹腔镜子宫肌瘤剔除术,可减少术中麻醉药物使用剂量,维持生命体征平稳及褪黑素水平。Objective To explore the application effect of Remifentanil in laparoscopic myomectomy under general anesthesia.Methods A total of 80 patients who underwent laparoscopic myomectomy under general anesthesia in Shouguang People’s Hospital from March 2021 to June 2023 were selected as the research objects.All the patients were divided into two groups using the random number table method,with 40 cases in each group.The control group was anesthetized with Cyclophorbol and Sevoflurane,while the observation group received continuous intravenous infusion of Remifentanil on the basis of the control group for anesthesia.The dosage of Cyclophorbol and Sevoflurane,mean arterial pressure,and melatonin levels in the two groups were compared.Results The dosage of Cyclophorbol and Sevoflurane in the observation group was less than that in the control group(P<0.05).The mean arterial pressure during anesthesia in the observation group was lower than that in the control group,and 10 minutes after anesthesia and during anesthesia resuscitation,the mean arterial pressure level in the observation group was higher than that in the control group(P<0.05).When anesthesia awakens,the melatonin level in the observation group was higher than that in the control group(P<0.05).Conclusion The application of Remifentanil in laparoscopic myomectomy under general anesthesia can reduce the dosage of anesthetic drugs,maintain the stability of vital signs and melatonin level.
