品管圈活动对肥胖初产妇剖宫产术后切口瘢痕形成的影响

摘　　要：目的探讨品管圈(QCC)活动对肥胖初产妇剖宫产术后切口瘢痕形成的影响。方法选取2020年4月至2023年9月山东省曹县第二人民医院收治的60例行剖宫产手术的肥胖初产妇,按抽签法分为对照组(30例)与干预组(30例)。对照组采用常规护理,干预组采用常规护理+QCC活动,比较两组产后恢复情况、切口并发症总发生率、切口瘢痕形成情况、切口瘢痕评分、护理总满意率。结果干预组产后首次排气时间、产后首次排便时间、产后首次下床活动时间以及住院时间均短于对照组,差异有统计学意义(P<0.05);干预组切口并发症总发生率低于对照组,差异有统计学意义(P<0.05);干预组瘢痕长度、宽度、厚度小于对照组,增生性瘢痕发生率低于对照组,差异有统计学意义(P<0.05);术后1、3个月,干预组温哥华瘢痕量表、瘢痕美容评估分级量表评分均低于对照组,差异有统计学意义(P<0.05);干预组护理总满意率高于对照组,差异有统计学意义(P<0.05)。结论QCC活动应用于肥胖初产妇剖宫产术后能够促进产妇恢复,改善切口瘢痕情况,值得推广应用。Objective To explore the Effect of quality control circle(QCC)activities on scar formation of the incision after cesarean section in obese primiparas.Methods Sixty obese primiparas who underwent cesarean section in Caoxian Second People’s Hospital in Shandong from April 2020 to September 2023 were selected and divided into control group(30 cases)and intervention group(30 cases)using lottery method.The control group received routine nursing,while the intervention group received routine nursing+QCC activities.The postpartum recovery,total incidence of incision complications,incision scar formation,scar score of incision,and total satisfaction rate of nursing were compared between the two groups.Results The intervention group had shorter postpartum first exhaust time,postpartum first defecation time,postpartum first get out of bed activity time,and hospitalization time compared with the control group,with statistically significant differences(P<0.05).The total incidence of incision complications in the intervention group was lower than that in the control group,and the difference was statistically significant(P<0.05).The length,width and thickness of scars in the intervention group were smaller than those in the control group,and the incidence rate of hypertrophic scars was lower than that in the control group,with statistically significant differences(P<0.05).At 1 and 3 months after surgery,the intervention group had lower Vancouver scar scale and scar cosmesis assessment and rating scores than the control group,with statistically significant differences(P<0.05).The total satisfaction rate of nursing in the intervention group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion The application of QCC activity in obese primiparas after cesarean section can promote the recovery of primiparas and improve the incision scar condition,which is worthy of popular ization and application.
