桑菊熏眼方联合睑板腺按摩治疗全飞秒激光术后干眼肝经郁热证临床研究

摘　　要：目的:探究中药桑菊熏眼方联合睑板腺按摩对全飞秒激光术(SMILE)后肝经郁热证干眼患者的效果。方法:纳入2023年5月—2023年12月东莞华厦眼科医院全飞秒SMILE术后2周诊断为干眼、且中医辨证为肝经郁热证的患者60例(120眼),按随机数字表法随机分为对照组和研究组,对照组患者30例(60眼)用玻璃酸钠滴眼液、氟米龙滴眼液滴眼,研究组30例(60眼)在对照组治疗基础上给予自拟桑菊熏眼方超声雾化熏蒸、睑板腺按摩治疗。2组均治疗30 d。记录两组患者治疗前后裸眼视力、眼表疾病指数(OSDI)量表评分、中医证候积分、泪液分泌试验(SchirmerⅠ试验)、泪膜破裂时间(NIBUT)、泪河高度、角膜荧光素钠染色(CFS)评分的数据及不良反应发生情况,进行比较。结果:两组患者治疗前上述指标比较,差异均无统计学意义(P>0.05)。治疗后两组患者上述指标均有所改善,且研究组优于对照组,差异均有统计学意义(P<0.05,P<0.01)。研究组总有效率(90.00%)高于对照组(70.00%),差异有统计学意义(P<0.01)。治疗期间,两组患者均无不良反应发生。结论:桑菊熏眼方联合睑板腺按摩治疗全飞秒SMILE术后肝经郁热证干眼临床疗效较好,可以显著改善眼部的不适症状,提高裸眼视力,延长泪膜破裂时间,促进泪液分泌,稳定泪膜,值得临床推广应用。Objective:To explore the effect of Chinese herb Sangju Xunan Formula combined with meibomian gland massage for dry eyes caused by stagnated heat of liver channel after SMILE surgery.Methods:60 patients(120 eyes)diagnosed as dry eyes caused by stagnated heat of liver channel after syndrome differentiation of TCM after 2 weeks of SMILE surgery who received treatment in Eye Hospital of Dongguan Huaxia from May 2023 to December 2023 were selected,and were randomly divided into control group and research group according to random number table method.30 patients(60 eyes)in the control group were given sodium hyaluronate eye drops and flumiron eye drops,and 30 patients(60 eyes)in the research group were given ultrasonic atomization fumigation with self-designed Sangju Xunan Formula and meibomian gland massage on the treatment basis of the control group.Patients in both groups were treated for 30 days.Before and after treatment,the uncorrected visual acuity,ocular surface disease index(OSDI)scale score,TCM symptom score,SchirmerⅠtest,tear film break-up time(NIBUT),height of the tear river,corneal fluorescein sodium staining(CFS)score and adverse reactions were recorded and compared between the two groups.Results:There was no significant difference in the above indicators between the two groups before treatment(P>0.05).After treatment,the above indexes of the two groups were improved,and the indexes of the research group were better than those of the control group,with the statistical differences(P<0.05,P<0.01).The total effective rate of the research group(90.00%)was higher than that of the control group(70.00%),and the difference was statistically significant(P<0.01).During the treatment,no adverse reactions occurred in the two groups.Conclusion:The combination of Sangju Xunan Formula and meibomian gland massage has a good clinical efficacy in the treatment of dry eyes caused by stagnated heat of liver channel after SMILE surgery.It can significantly improve the symptoms of eye discomfort,improve the uncorrected visua
