摘　　要：胆与胃皆为六腑,位居中焦,二者在生理、病理方面均有较为密切的关系,胆胃不和可致胃气上逆,产生呕吐。《灵枢·四时气》指出“呕胆”的病机为胆液泄、胃气逆,即“邪在胆,逆在胃”。从胆与胃的生理、病理及经络之间的相互关系出发,探究呕胆的病因病机,并与伤寒少阳病以及肝胆同病等进行比较研究。呕胆与现代医学胆汁反流性胃炎高度相似,中医药治疗呕胆分从气论治、从热论治、从毒论治,为临床治疗胆汁反流性胃炎提供治疗思路和方法。未来研究将聚焦于完善中医辨证分型、深入探究发病机制及治疗机理,以提高临床疗效。Both gallbladder and stomach belong to the six fu-organs,located in the middle energizer,and they are closely related to each other in physiology and pathology.Disharmony between gallbladder and stomach can cause stomach-Qi to reverse,producing vomiting.According to Qi of Four Seasons of Miraculous Pivot,the pathogenesis of“gallbladder bile vomiting”is the abnormal discharge of bile fluid and the inverse of stomach-Qi,that is,gallbladder and stomach comorbidity.This study explores the etiology and pathogenesis of gallbladder bile vomiting from the the physiology,pathology and the relationship between the meridians of gallbladder and stomach,and compares it with typhoid Shaoyang disease and liver-gallbladder comorbidity.Gallbladder bile vomiting is highly similar to bile reflux gastritis in modern medicine.The treatment of gallbladder bile vomiting with traditional Chinese medicine is based on Qi,heat and toxicity,which provides therapeutic ideas and methods for clinical treatment of bile reflux gastritis.Future research will focus on improving the TCM syndrome differentiation and exploring the pathogenesis and treatment mechanism to improve the clinical efficacy.
