摘　　要：目的探究鼻窦CT在诊断嗜酸性细胞性慢性鼻-鼻窦炎伴鼻息肉(Eosinophilic chronic rhinosinusitis with nasal polyps,eCRSwNP)中的价值。方法选CRSwNP患者120例(2020.01至2020.12收治),其中Non-eCRSwNP患者有50例,eCRSwNP有70例,记录患者基本特征。入组患者均于鼻内镜鼻窦炎手术前7d,接受鼻窦CT、外周血常规检查等。探究鼻窦CT的应用价值。结果eCRSwNP组EOC计数、EOC计数异常升高率、EOC百分比、EOC百分比异常升高率比Non-eCRSwNP组高(P<0.05);eCRSwNP组两侧额窦、蝶窦、前组筛窦、上颌窦、E/M比值及总体Lund-Mackay评分比Non-eCRSwNP组高(P<0.05);用Enter法多因素二分类Logistics回归分析,结果显示,外周血EOS百分比、lund-Mackay评分是预测eCRSwNP的影响因素(P<0.05);外周血EOS百分比联合lund-Mackay评分的ROC曲线下面积为0.904(95% CI:0.827-0.988),敏感度、特异度是90.90%、90.30%。结论鼻窦CT在诊断嗜酸性细胞性慢性鼻-鼻窦炎伴鼻息肉患者应用一定价值,值得优选。Objective To explore the value of sinus CT in the diagnosis of eosinophilic chronic rhinosinusitis with nasal polyps(eosinophilic chronic rhinosinusitis with nasal polyps,eCRSwNP).Methods Of 120 CRSwNP patients(admitted from 2020.01 to 2020.12),including 50 Non-eCRSwNP patients and 70 eCRSwNP patients,the basic patient characteristics were recorded.All the enrolled patients were treated 7d before the nasal endoscopic sinusitis surgery and underwent sinus CT and routine peripheral blood examination.To explore the application value of sinus CT.Results The EOC count,EOC percentage were higher than the Non-eCRSwNP group(P<0.05);The bilateral frontal lunuses,sphenoid sinus,anterior ethid sinus,maxillary sinus,E/M ratio and overall Lund-Mackay score were higher than those of the Non-eCRSwNP group(P<0.05);Multivariate dichotomy Logistics regression analysis by the Enter method,result display,Percent EOS of peripheral blood and lun-Mackay score were the influencing factors in predicting eCRSwNP(P<0.05);The area under the ROC curve of the percent EOS in peripheral blood combined with the lund-Mackay score is 0.904(95%CI:0.827-0.988),Sensitivity and specificity were 90.90%and 90.30%.Conclusion Sinus CT is of some value in the diagnosis of eosinophilic chronic rhinosinusitis with nasal polyps.
