摘　　要：目的系统评价卡泊三醇治疗白癜风的有效性和安全性,并分析其机理。方法计算机检索中国生物医学文献光盘数据库及中国期刊全文数据库,纳入比较卡泊三醇治疗白癜风的随机对照试验(RCT),对纳入研究的方法学质量进行评价,并应用Rev Man5.3软件进行统计分析。结果共纳入7项RCT,合计906例患者。Meta分析结果表明,卡泊三醇配合光疗及外用药物治疗可有效治疗白癜风,总有效率[OR=4.80,95%CI 3.41~6.77,Z=8.96,P<0.00001],差异有统计学意义;两组的不良反应[OR=0.22,95%CI 0.05~0.94,Z=2.05,P=0.04],差异有统计学意义。结论卡泊三醇治疗白癜风可能是通过逆转调节性T细胞的缺乏,抑制细胞毒性T细胞应答而起到治疗作用。Objective To systematically evaluate the effectiveness and safety of calcipotriol in the treatment of vitiligo and to investigate the mechanism.Methods We searched CBM and CNKI,randomized controlled trials(RCT)on calcipotriol in the treatment of vitiligo were included and compared.The methodological quality of included studies was evaluated and meta-analysis was performed by Rev Man 5.3 software.Results Seven RCTs with a total of 906 patients were included.Meta-analysis showed that calcipotriol could treat the vitiligo.There were statistical differences in total efficiency[OR=4.80,95%CI 3.41-6.77,Z=8.96,P<0.00001]and the adverse drug reaction in two groups[OR=0.22,95%CI 0.05-0.94,Z=2.05,P=0.04].Conclusion The treatment of vitiligo with calcipotriol may play a therapeutic role by reversing the lack of regulatory T cells and inhibiting the response of cytotoxic T cells.
