安宁疗护视野下家庭参与式护理干预对生命终末期血液肿瘤患儿负性情绪及生活质量的影响

摘　　要：目的观察安宁疗护视野下的家庭参与式护理干预措施对生命终末期血液肿瘤患儿负性情绪及生活质量的影响。方法选择2020年8月至2023年8月住院的血液肿瘤患儿80例,随机数字表法将其分为研究组和对照组,每组40例。对照组患儿予以常规护理,研究组患儿在此基础上予以安宁疗护视野下的家庭参与式护理干预措施。采用儿童焦虑性情绪障碍筛查量表(SCARED)、儿童抑郁障碍自评量表(DSRSC)、儿童生活质量普适性核心量表(PedsQL4.0)评估干预效果。结果干预前,2组患儿SCARED量表各维度评分及总分、DSRSC量表评分、PedsQL4.0量表各维度评分及总分比较,差异均无统计学意义(P>0.05);干预后,研究组患儿SCARED量表各维度评分及总分、DSRSC量表评分均低于对照组,PedsQL4.0量表各维度评分及总分高于对照组,差异均有统计学意义(P<0.05)。结论安宁疗护视野下的家庭参与式护理干预可有效缓解生命终末期肿瘤患儿焦虑及抑郁情绪,提高患儿生活质量。Objective To observe the effect of family participatory nursing intervention on negative emotions and quality of life in children with terminal stage of hematological malignancy(HM)under view of palliative care.Methods Eighty HM children admitted to our hospital from August 2020 to August 2023 were enrolled.They were divided into a control group(routine nursing care,n=40)and a study group(routine nursing care+family participatory nursing intervention under view of palliative care,n=40)according to a random number table.The intervention effects were evaluated using the Screen for Child Anxiety Related Emotional Disorders(SCARED),the Depression Self-Rating Scale for Children(DSRSC),and the Pediatric Quality of Life Inventory 4.0(PedsQL4.0).Results Before the intervention,there were no significant differences in the scores of the SCARED,DSRSC,and PedsQL4.0 between the two groups(P>0.05).After the intervention,the scores of the SCARED and DSRSC in the study group were significantly lower than those in the control group,while the scores of the PedsQL4.0 in the study group were significantly higher(all P<0.05).Conclusion Family participatory nursing intervention under view of palliative care can effectively alleviate anxiety and depression and improve their quality of life in children with terminal stage of HM.
