摘 要:目的研究硫辛酸联合己酮可可碱对老年糖尿病周围神经病变(DPN)患者神经传导速度的影响。方法前瞻性选取2022年1月至2023年12月河南(郑州)现代医学研究院中医院收治的老年DPN患者92例,以随机数字表法分为联合组48例、单一组44例。单一组采用己酮可可碱治疗,联合组采用硫辛酸联合己酮可可碱治疗。比较两组临床疗效、正中神经F波、多伦多临床评分系统(TCSS)及密歇根神经病变筛选表(MNSI)评分、氧化应激指标、不良反应总发生率。结果联合组总有效率(93.75%)高于单一组(77.27%)(P<0.05),且两组不良反应总发生率比较,差异无统计学意义(P>0.05);治疗后两组正中神经F波、氧化应激指标、TCSS及MNSI评分均改善,且联合组优于单一组(P<0.05)。结论硫辛酸联合己酮可可碱治疗老年DPN患者效果显著,可有效地提高神经传导速度,减轻氧化应激反应,且具有一定安全性。Objective To study the effect of thioctic acid combined with pentoxifylline on nerve conduction velocity in patients with diabetic peripheral neuropathy(DPN).Methods Ninety-two elderly patients with DPN were selected and treated in Henan(Zhengzhou)Modern Medical Research Institute Traditional Chinese Medicine Hospital from January 2022 to December 2023.They were randomly divided into a combined group of 48 patients and a single group of 44 patients using a random number table method.The single group was treated with pentoxifylline,while the combined group was treated with thioctic acid combined with pentoxifylline.The clinical efficacy,median nerve F-wave,Toronto Clinical Scoring System(TCSS)and Michigan Neuropathy Screening Instrument(MNSI)scores,oxidative stress indicators,and total incidence of adverse reactions were compared between the two groups.Results The total effective rate of the combined group(93.75%)was higher than that of the single group(77.27%)(P<0.05),and there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05);after treatment,the F-wave,oxidative stress index,TCSS,and MNSI scores of the two groups improved,and the combined group was superior to the single group(P<0.05).Conclusion Thioctic acid combined with pentoxifylline in the treatment of elderly patients with DPN has significant effect,can effectively improve nerve conduction velocity,reduce oxidative stress reaction,and has certain safety.
