醒神开窍针结合通窍活血汤在重型颅脑损伤患者术后康复中的应用

摘　　要：目的观察醒神开窍针结合通窍活血汤对重型颅脑损伤术后康复情况的影响。方法本研究为前瞻性研究,研究对象选取南阳医学高等专科学校第一附属医院2020年6月至2023年9月期间收治的90例重型颅脑损伤患者。经计算机随机分组法将其分别列为西医组和中医组,各45例。西医组实施常规西医治疗,中医组采用醒神开窍针结合通窍活血汤治疗,比较两组神经炎症、氧化应激反应、神经功能恢复情况,以及预后情况。结果在不同治疗方案下,中医组的白细胞介素-8(IL-8)、单核细胞趋化蛋白-1(MCP-1)、超敏C-反应蛋白(hs-CRP)均低于西医组;中医组的超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)均高于西医组,丙二醛(MDA)低于西医组;中医组的半乳糖凝集素3(Galectin-3)、粒细胞巨噬细胞-集落刺激因子(GM-CSF)、神经肽Y(NPY)均低于西医组;中医组的格拉斯哥预后指数(GOS)高于西医组,改良Fisher量表(mFS)评分低于西医组,差异有统计学意义(P<0.05)。结论醒神开窍针结合通窍活血汤能减轻重型颅脑损伤的神经炎症及氧化应激反应,对促进患者神经功能恢复,并改善预后情况均有积极意义。Objective Observing the effect of the combination of xingshen kaiqiao needle with tongqiao huoxue decoction on postoperative rehabilitation of severe craniocerebral injury.Methods This study was a prospective study,selected 90 patients with severe craniocerebral injury admitted to The First Affiliated Hospital of Nanyang Medical College from June 2020 to September 2023,divided into a Western medicine group(45 cases)and a Chinese medicine group(45 cases)using computer random grouping method.The Western medicine group received routine Western medicine treatment,while the Chinese medicine group received xingshen kaiqiao needle combined with tongqiao huoxue decoction treatment.The neuroinflammation,oxidative stress response,neurological function recovery,and prognosis of the two groups were compared.Results Under different treatment plans,the levels of IL-8,MCP-1,and hs CRP in the Chinese medicine group were lower than the Western medicine group;the SOD and GSH Px in the Chinese medicine group were higher than the Western medicine group.MDA was lower than the Western medicine group;the Galectin-3,GM-CSF,and NPY of the Chinese medicine group were lower than the Western medicine group;the GOS score of the Chinese medicine group was higher than that of the Western medicine group,and the mFS score was lower than that of the Western medicine group(P<0.05).Conclusion The combination of xingshen kaiqiao needle with tongqiao huoxue decoction can alleviate neuroinflammation and oxidative stress response in severe craniocerebral injury,promote the recovery of neurological function in patients,and improve prognosis.
