摘  要:目的 探讨3D-DSA与3D-CTA对颅内动脉瘤的诊断效能。方法 回顾性分析2022年2月-2023年2月天津环湖医院收治的83例疑似颅内动脉瘤患者相关资料,以血管介入或外科开颅手术结果作为金标准,比较3D-DSA与3D-CTA对颅内动脉瘤的诊断效能,并进一步分析其对动脉瘤大小的分辨能力。结果 83例疑似颅内动脉瘤患者,经血管介入或外科开颅手术最终确诊阳性80例、阴性3例。其中,瘤体直径<3mm37例,瘤体直径3~5 mm 43例。3D-DSA检查结果显示,阳性79例、阴性4例;其中,真阳性78例、真阴性2例。3D-CTA检查结果显示,阳性72例、阴性11例;其中,真阳性70例、真阴性2例。两种检查方式阳性率、阴性率、灵敏度以及特异度比较,差异无统计学意义(P>0.05)。3D-DSA检查颅内动脉瘤的诊断符合率高于3D-CTA,差异有统计学意义(P<0.05)。3D-DSA对直径3 mm以下动脉瘤的检出率高于3D-CTA,差异有统计学意义(P<0.05)。两种检查方式对直径为3~5 mm动脉瘤的检出率比较,差异无统计学意义(P>0.05)。结论 3D-DSA在颅内动脉瘤的诊断中具有更高的效能,提升了颅内动脉瘤的检出率,为临床诊断和治疗提供了更加可靠的科学依据。Objective To explore the diagnostic efficacy of 3D-DSA and 3D-CTA for intracranial aneurysm.Methods Eighty-three suspected intracranial aneurysm patients admitted to Tianjin Huanhu Hospital from February 2022 to February 2023 were analized retrospectively.Using vascular intervention or surgical craniotomy results as the gold standard,the diagnostic efficacy of 3D-DSA and 3D-CTA for intracranial aneurysms were compared,and their ability to distinguish aneurysm size were analized further.Results 83 patients with suspected intracranial aneurysms were finally diagnosed positively in 80 cases and negatively in 3 cases through vascular intervention or surgical craniotomy,of which 37 cases had aneurysms with a diameter of less than 3 mm and 43 cases had aneurysms with a diameter of 3~5 mm.3D-DSA test results showed that 79 cases were positive,and 4 cases were negative,of which 78 cases were true positives and 2 cases were true negatives.3D-CTA test results showed that 72 cases were positive and 11 cases were negative,of which 70 cases were true positive and 2 cases were true negative.There was no difference in the positivity rate,negativity rate,sensitivity,and specificity of the two tests(P>0.05),and the diagnostic conformity rate of intracranial aneurysms detected by 3D-DSA was significantly higher than that of 3D-CTA(P<0.05).The detection rate of 3D-DSA was significantly higher than that of 3D-CTA for diameters of less than 3 mm(P<0.05).And there was no difference in the detection rate of diameters of 3~5 mm between the two detection modalities(P>0.05).Conclusion 3D-DSA showed higher efficacy in the diagnosis of intracranial aneurysm,which could significantly improve the detection rate of intracranial aneurysm,and provide a more reliable scientific basis for clinical diagnosis and treatment.
