摘 要:目的 探讨血清肌钙蛋白I(cardiac troponin I,cTnI)联合心型脂肪酸结合蛋白(heart-type fatty acid binding protein,H-FABP)检测对新生儿窒息后心肌损害的诊断价值。方法 选择2020年1月-2022年12月在丹阳市妇幼保健院产科出生的新生儿窒息病例80例进行研究,根据窒息后是否出现心肌损害分为心肌损害组28例和无心肌损害组52例。采用多因素logistic回归分析筛选新生儿窒息后心肌损害的危险因素,评估血清cTnI和H-FABP指标单一检测及联合检测对其的诊断价值。结果 心肌损害组的cTnI、H-FABP水平均高于无心肌损害组,差异有统计学意义(P<0.05)。经logistic回归模型分析得出,cTnI、H-FABP水平升高是新生儿窒息后发生心肌损害的危险因素(P<0.05)。联合检测对新生儿窒息后心肌损害的诊断效能高于各指标单独检测(P<0.05)。结论 血清cTnI和H-FABP水平的变化对新生儿窒息后心肌损害的诊断具有重要价值,联合检测两项指标可以提高新生儿窒息后心肌损害的诊断效能。Objective To investigate the diagnostic value of combined detection of serum troponin I(cTnI)and heart-type fatty acid-binding protein(H-FABP)in myocardial injury after neonatal asphyxia.Methods A study was conducted on 80 cases of asphyxia infants born in the obstetrics department of Danyang Maternal and Child Health Hospital from January 2020 to December 2022.Based on whether myocardial damage occurred in newborns after asphyxia,they were divided into a myocardial damage group of 28 cases and a non myocardial damage group of 52 cases.The risk factors for myocardial damage in newborns after asphyxia were screened by multiple logistic regression analysis,and the diagnostic value of single and combined detection of serum cTnI and H-FABP indicators was evaluated.Results The levels of cTnI and H-FABP in the myocardial injury group were higher than those in the non-myocardial injury group(P<0.05).The logistic regression model analysis revealed that increased levels of cTnI and H-FABP were risk factors for myocardial injury after neonatal asphyxia(P<0.05).The diagnostic efficiency of combined detection for myocardial injury after neonatal asphyxia was higher than that of individual detection of each indicator(P<0.05).Conclusion The changes in serum cTnI and H-FABP levels have important diagnostic value for myocardial injury after neonatal asphyxia,and the combined detection of these two indicators can improve the diagnostic efficiency of myocardial injury after neonatal asphyxia.
