立体定向微创颅内血肿清除术联合尿激酶治疗高血压脑出血患者的效果分析

摘　　要：目的:探讨立体定向微创颅内血肿清除术联合尿激酶在高血压脑出血患者治疗中的效果。方法:选取2020年1月1日-2023年12月31日临沭县人民医院收治的80例高血压脑出血患者为研究对象,依据方便抽样法随机分为对照组和研究组,各40例。对照组采用传统开颅血肿清除术治疗,研究组采用立体定向微创颅内血肿清除术联合尿激酶治疗,比较两组的治疗效果、神经细胞损伤因子及炎症因子水平和并发症发生情况。结果:试验组治疗总有效率为95.00%,高于对照组的80.00%,差异有统计学意义(P<0.05)。两组术后第1天的神经细胞损伤因子及炎症因子水平对比,差异均无统计学意义(P>0.05);术后第7天,研究组TNF-α、MMP-9、hs-CRP水平均低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.50%,低于对照组的15.00%,差异有统计学意义(P<0.05)。结论:立体定向微创颅内血肿清除术联合尿激酶治疗高血压脑出血,效果显著,可减少脑组织炎症反应,有利于患者神经功能恢复,且并发症发生率低。Objective:To investigate the effect of stereotactic minimally invasive removal of intracranial hematoma combined with urokinase in the treatment of patients with hypertensive cerebral hemorrhage.Methods:A total of 80 patients with hypertensive cerebral hemorrhage treated in Linshu County People’s Hospital from January 12020 to December 312023 were selected as the study objects,and randomly divided into a control group and a study group according to convenient sampling method,with 40 cases in each group.The control group was treated with traditional craniotomy hematoma removal,and the study group was treated with stereotactic minimally invasive intracranial hematoma removal combined with urokinase treatment.The therapeutic effect,the levels of nerve cell injury factors and inflammatory factors and the incidence of complications were compared between the two groups.Results:The total effective rate of the study group was 95.00%,which was higher than 80.00%of the control group,the difference was statistically significant(P<0.05).On the 7th day after surgery,the levels of TNF-α,MMP-9 and hs-CRP in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The complication rate of the study group was 2.50%,lower than 15.00%of the control group,the difference was statistically significant(P<0.05).Conclusion:Stereotactic minimally invasive removal of intracranial hematoma combined with urokinase is effective in the treatment of hypertensive intracerebral hemorrhage,which can reduce the inflammatory response of brain tissue,is conducive to the recovery of neurological function,and has low incidence of complications.
