龙胆泻肝汤加减结合火针对肝经郁热证带状疱疹性神经痛的临床疗效观察及对补体表达的影响

摘　　要：目的:探讨龙胆泻肝汤加减结合火针对肝经郁热证带状疱疹性神经痛的临床疗效及对补体表达的影响。方法:选取江西省中西医结合医院2022年10月—2023年10月收治的60例肝经郁热证带状疱疹性神经痛患者,随机分为两组。对照组(n=30)给予龙胆泻肝汤加减治疗,观察组(n=30)在对照组基础上联合火针治疗。比较两组临床疗效,治疗前后中医症候积分、疼痛程度、免疫指标。结果:观察组治疗总有效率较对照组高(P<0.05)。观察组治疗后中医症候积分及现时疼痛强度(PPI)评分均较对照组低(P<0.05)。观察组治疗后CD4^(+)、CD4^(+)/CD8^(+)、补体C3、补体C4水平均较对照组高,CD8^(+)水平较对照组低(P<0.05)。结论:龙胆泻肝汤加减联合火针治疗肝经郁热证带状疱疹性神经痛可有效提高效果,减轻患者神经痛症状,并改善免疫指标。Objective:To investigate the clinical efficacy of modified Longdan Xiegan Decoction combined with fire needle in herpetic neuralgia with pattern of heat stagnation in liver channel and its influence on complement expression.Method:A total of 60 patients with herpetic neuralgia with pattern of heat stagnation in liver channel admitted to Jiangxi Integrated Traditional Chinese and Western Medicine Hospital from October 2022 to October 2023 were randomly divided into two groups.The control group(n=30)was treated with modified Longdan Xiegan Decoction,and the observation group(n=30)was treated with fire needle on the basis of the control group.The clinical efficacy,TCM syndromes integrations,pain degree and immune indexes before and after treatment were compared between two groups.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05).TCM symptoms integrations and present pain intensity(PPI)scores after treatment in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CD4^(+),CD4^(+)/CD8^(+),complement C3 and complement C4 in observation group were higher than those in control group,and the level of CD8^(+)was lower than that in control group(P<0.05).Conclusion:Modified Longdan Xiegan Decoction combined with fire needle can effectively improve the effect in herpetic neuralgia with pattern of heat stagnation in liver channel,relieve the symptoms of neuralgia and improve the immune indexes.
