甲氨蝶呤联合腹腔镜手术对宫外孕患者的临床研究

摘　　要：目的:探究甲氨蝶呤联合腹腔镜手术对于宫外孕患者人绒毛膜促性腺激素(hCG)下降率和宫内妊娠率的影响。方法:按照随机数字表法将抚州市妇幼保健院妇科2022年6月—2023年6月收治的宫外孕患者102例分为研究组(51例)和对照组(51例)。对照组行腹腔镜手术治疗,在对照组基础上,研究组加用甲氨蝶呤治疗,比较两组hCG水平、hCG下降率和恢复正常时间、临床疗效、包块消失时间、住院时间及月经恢复时间、不良反应发生情况和术后1年内宫内妊娠情况。结果:研究组治疗后hCG水平低于对照组,hCG恢复正常时间短于对照组,hCG下降率高于对照组,差异均有统计学意义(P<0.05);研究组治疗后总有效率高于对照组,差异有统计学意义(P<0.05);研究组包块消失时间、住院时间及月经恢复时间均较短于对照组,差异均有统计学意义(P<0.05);两组患者治疗后不良反应发生率比较,差异无统计学意义(P>0.05);研究组术后1年宫内妊娠率高于对照组,差异有统计学意义(P<0.05)。结论:宫外孕患者采用甲氨蝶呤联合腹腔镜手术治疗,能够有效降低hCG水平,缩短hCG恢复正常时间,加快患者恢复,利于提高宫内妊娠率、hCG下降率,效果确切,且安全性较高,值得推广应用。Objective:To explore the effect of Methotrexate combined with laparoscopic surgery on the decrease rate of human chorionic gonadotropin(hCG)and intrauterine pregnancy rate in extrauterine pregnancy.Method:According to the method of random number table,102 extrauterine pregnancy patients admitted to the Gynecology Department of Fuzhou Maternal and Child Health Hospital from June 2022 to June 2023 were divided into study group(51 cases)and control group(51 cases).The control group was treated with laparoscopic surgery.On the basis of the control group,the study group was treated with Methotrexate.The hCG level,the decrease rate of hCG and time for hCG to return to normal,clinical efficacy,time of mass disappearance,hospital stay,menstrual recovery time,adverse reactions and intrauterine pregnancy within 1 year after surgery were compared between the two groups.Result:After treatment,the hCG level in the study group was lower than that in the control group,the recovery time of hCG was shorter than that in the control group,and the hCG decreasing rate was higher than that in the control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).The time of mass disappearance,hospital stay and menstrual recovery in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The rate of intrauterine pregnancy 1 year after surgery in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of patients with ectopic pregnancy with Methotrexate combined with laparoscopic surgery can effectively reduce the hCG level,shorten the time for hCG to return to normal,and accelerate the recovery of patients,which is conducive to improving the intrauterine pr
