摘　　要：目的探究孕早期医学营养治疗对妊娠期糖尿病高危孕妇胰岛功能及同型半胱氨酸(HCY)、脂联素(APN)水平的影响。方法择取2022年1月至2023年1月收治的84例妊娠期糖尿病高危孕妇为研究对象,将其随机分为对照组和观察组,各42例。对照组采用常规孕期保健指导,观察组在对照组基础上增加孕早期医学营养治疗。比较两组的治疗效果。结果治疗后,观察组的糖化血红蛋白(HbA1c)、空腹血糖(FPG)、果糖胺(FMN)、空腹C肽(FCP)水平低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的HCY水平低于对照组,APN水平高于对照组,差异具有统计学意义(P<0.05)。观察组的母婴不良结局总发生率低于对照组,差异具有统计学意义(P<0.05)。结论孕早期医学营养治疗不仅能够改善妊娠期糖尿病高危孕妇胰岛功能,调节HCY、APN水平,还能降低母婴不良结局发生风险,值得推广。Objective To explore the effects of early pregnancy medical nutrition therapy on islet function and homocysteine(HCY)and adiponectin(APN)levels in high-risk pregnant women with gestational diabetes mellitus.Methods A total of 84 pregnant women with high risk of gestational diabetes mellitus admitted from January 2022 to January 2023 were randomly divided into control group and observation group,with 42 cases in each group.The control group was given routine pregnancy care guidance,and the observation group was given early pregnancy medical nutrition therapy on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the levels of glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG),fructosamine(FMN)and fasting C peptide(FCP)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the HCY level of the observation group was lower than that of the control group,and the APN level was higher than that of the control group,and the differences were statistically significant(P<0.05).The total incidence of maternal and infant adverse outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Early pregnancy medical nutrition therapy can not only improve the islet function of high-risk pregnant women with gestational diabetes mellitus,regulate the levels of HCY and APN,but also reduce the risk of adverse maternal and infant outcomes,which is worthy of promotion.
