摘 要:目的:探讨全内技术与由外向内移植物引入技术重建前交叉韧带(anterior cruciate ligament,ACL)的效果。方法:选择2021年1月—2023年7月于九江市第一人民医院接受治疗的87例ACL损伤患者,根据随机数字表法分为研究组(n=43)与对照组(n=44)。研究组采用全内技术重建ACL,对照组采用由外向内移植物引入技术重建ACL,比较两组围手术期指标、疼痛情况、膝关节功能及术后1年隧道扩大程度。结果:两组手术耗时比较,差异无统计学意义(P>0.05);研究组肌腱切取数量少于对照组,移植物直径大于对照组,差异均有统计学意义(P<0.05)。术后24 h、2周,研究组视觉模拟评分法(visual analogue scale,VAS)评分均低于对照组,差异均有统计学意义(P<0.05);术后6个月、1年,两组VAS评分比较,差异均无统计学意义(P>0.05)。术后6个月、1年,两组Lysholm膝关节评分均高于术前,差异均有统计学意义(P<0.05),但两组间比较,差异均无统计学意义(P>0.05)。研究组胫骨隧道扩大率低于对照组,差异有统计学意义(P<0.05)。术后1年,两组前抽屉试验和Lachman试验结果比较,差异均无统计学意义(P>0.05)。结论:全内技术与由外向内移植物引入技术应用于重建ACL均能改善患者的膝关节功能,但全内技术的术后早期疼痛程度较轻,胫骨隧道扩大率低。Objective:To investigate the effect of the all-inside technique and the transplant introduction from outside to inside technology on the reconstruction of the anterior cruciate ligament(ACL).Method:A total of 87 patients with ACL injury who received treatment in Jiujiang NO.1 People's Hospital from January 2021 to July 2023 were selected and divided into study group(n=43)and control group(n=44)according to random number table method.The study group used all-inside technique to reconstruct ACL,while the control group used transplant introduction from outside to inside technology to reconstruct ACL.Perioperative indexes,pain,knee function and tunnel enlargement 1 year after surgery were compared between the two groups.Result:There was no significant difference in surgery time between the two groups(P>0.05).The number of tendon resection in the study group was less than that in the control group,and the graft diameter was larger than that in the control group,the differences were statistically significant(P<0.05).At 24 h and 2 weeks after surgery,the visual analogue scale(VAS)score of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).At 6 months and 1 year after surgery,there were no significant differences in VAS scores between the two groups(P>0.05).At 6 months and 1 year after surgery,Lysholm knee joint scores in two groups were higher than those before surgery,the differences were statistically significant(P<0.05),but there were no statistical significance between two groups(P>0.05).The enlargement rate of tibial tunnel in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).At 1 year after surgery,there were no significant differences in the results of the front drawer test and Lachman test between the two groups(P>0.05).Conclusion:The application of all-inside technique and transplant introduction from outside to inside technology in ACL reconstruction can improve the knee joint function of
