摘 要:目的:探讨多模态超声联合颈项透明层(NT)测量、头臀长对孕中期疑似心脏畸形胎儿的诊断价值。方法:回顾102例2022年1月—2023年12月抚州市立医院收治的孕中期疑似心脏畸形胎儿的孕妇,孕中期均行多模态超声和NT、头臀长检查,以引产后胚胎病理解剖及随访结果为判断胎儿心脏畸形的金标准,分析多模态超声和NT测量、头臀长诊断孕中期心脏畸形胎儿的价值。结果:金标准诊断结果显示,102例孕中期疑似心脏畸形胎儿中有32例心脏畸形胎儿。心脏畸形胎儿头臀长及NT厚度别为(68.65±8.54)、(7.86±1.32)mm。非心脏畸形胎儿头臀长及NT厚度别为(56.43±5.52)、(1.54±0.30)mm。心脏畸形胎儿头臀长及NT厚度均高于非心脏畸形胎儿,差异均有统计学意义(P<0.05)。多模态超声与金标准结果的一致性较好(Kappa=0.697,P<0.05);NT与金标准结果的一致性极好(Kappa=0.824,P<0.05);头臀长与金标准结果的一致性较好(Kappa=0.721,P<0.05);联合诊断与金标准诊断结果一致性极好(Kappa=0.910,P<0.05)。多模态超声联合NT、头臀长对孕中期疑似心脏畸形胎儿的准确度较单一诊断高(P<0.05)。结论:多模态超声联合NT、头臀长对孕中期疑似心脏畸形胎儿的诊断价值较高。Objective:To explore the diagnostic value of multimodal ultrasound combined with nuchal translucency(NT)measurement and head-buttocks length in fetuses with suspected cardiac malformations in the second trimester.Method:A review of 102 pregnant women with suspected cardiac malformations admitted to Fuzhou Municipal Hospital from January 2022 to December 2023 in the second trimester were reviewed,and multimodal ultrasound,NT and head-buttocks length examinations were performed in the second trimester,and the results of embryo pathological anatomy and follow-up after induction of labor were used as the gold standard for judging fetal cardiac malformations,and the value of multimodal ultrasound and NT measurement and head-buttocks length in diagnosing fetuses with cardiac malformations in the second trimester was analyzed.Result:Gold standard diagnostic results showed that 32 fetuses had suspected cardiac malformation in the second trimester.The fetal headbuttocks length and NT thickness of cardiac malformationswere(68.65±8.54)mm and(7.86±1.32)mm.The fetal head-buttocks length and NT thickness of non-cardiac malformations were(56.43±5.52)mm and(1.54±0.30)mm.The lengths of head and NT thickness were higher than those with non-cardiac malformations(P<0.05).The results of multimodal ultrasound were in good agreement with the gold standard(Kappa=0.697,P<0.05).The results of NT were in excellent agreement with the gold standard(Kappa=0.824,P<0.05).The head-buttocks length was consistent with the gold standard results(Kappa=0.721,P<0.05).The results of the combined diagnosis were in good agreement with the gold standard diagnosis(Kappa=0.910,P<0.05).The accuracy of multimodal ultrasound combined with NT and head-buttocks length in fetuses with suspected cardiac malformations in the second trimester was higher than that of a single diagnosis(P<0.05).Conclusion:The multimodal ultrasound combined with NT,head-buttocks length has a high diagnostic value for fetuses with suspected cardiac malformations in the second trimes
