他汀类药物诱发新发糖尿病风险的研究进展

摘　　要：一直以来,他汀类药物是治疗心血管疾病的一线用药,但有研究表明长期应用可能会带来诸多不良效应。本篇综述主要探讨他汀类药物诱发新发糖尿病的相关因素及发病机制,具体包括影响胰岛素分泌、胰岛素敏感性与抵抗性、影响miRNA及β细胞线粒体功能等。虽然他汀类药物可能会导致新发糖尿病,但临床上仍应该鼓励患者使用他汀类药物来预防心血管疾病,如没有禁忌,还可以联用非他汀类降脂药物来减轻他汀类药物带来的不良效应,同时医生应测量药物开始使用后的血糖状态,并根据结果定制生活方式建议和护理。希望本篇综述能够为糖尿病患者的个体化诊治提供指导意义。Statins have long been the first-line drug for the treatment of cardiovascular disease,but studies have shown that long-term use will bring many adverse effects.This review focuses on the related factors and pathogenesis of new-onset diabetes mellitus induced by statins,including the effects on insulin secretion,insulin sensitivity and resistance,miRNA andβcell mitochondrial function.Although statins may lead to new-onset diabetes mellitus,patients should be encouraged to use statins to prevent cardiovascular disease in the clinic,and if there is no contraindication,they can also be combined with non-statin lipid-lowering drugs to reduce the adverse effects of statins,and doctors should measure blood sugar status after starting the drug and customize lifestyle recommendations and care based on the results.It is hoped that this review can provide guidance for the individualized diagnosis and treatment of diabetic patients.
