摘 要:目的探讨吲哚菁绿血管造影(ICGA)与红外热成像技术在评估截指平面中的临床应用价值。方法选取自2021年12月至2023年12月首都医科大学附属北京积水潭医院烧伤整形科收治的18例因外伤或糖尿病导致的肢体血运障碍而考虑需要截指的患者为研究对象。所有患者在手术治疗前均接受ICGA技术及红外热成像仪评估。根据术中所见及最终保肢的长度,评估两种方法的截指决策,以及截指平面预测是否准确。结果18例患者共有32根手指需明确截指平面,其中,有27根手指接受了不同平面的截指手术,5根手指未行截指手术。ICGA的截指决策准确性为90.6%(29/32),红外成像为84.4%(27/32)。在截指结局的27根手指中,ICGA的截指平面预测准确性为88.9%(24/27),红外成像为92.6%(25/27)。结论ICGA与红外热成像技术在评估截指平面中均具有较高的准确性,能够为临床决策提供科学依据。Objective To investigate the clinical application value of indocyanine green angiography(ICGA)and infrared thermal imaging in the evaluation of intercept plane.Methods A total of 18 patients considered for finger amputation due to limb blood circulation disorders caused by trauma or diabetes were selected as the study objects from December 2021 to December 2023 in the Department of Burn and Plastic Surgery,Beijing Jishuitan Hospital Affiliated to Capital Medical University.All patients were evaluated by ICGA technology and infrared thermal imager prior to surgical treatment.Based on intraoperative findings and final limb salvage length,the two methods were used to evaluate the accuracy of finger amputation decisions and the prediction of finger amputation plane.Results In 18 patients,a total of 32 fingers needed to be determined on the plane of finger amputation.Among them,27 fingers underwent finger amputation on different planes,and 5 fingers did not undergo finger amputation.The accuracy of ICGA′s truncated decision was 90.6%(29/32)and 84.4%(27/32)for infrared imaging.Among the 27 fingers in the amputation outcome,the prediction accuracy of the amputation plane was 88.9%(24/27)for ICGA and 92.6%(25/27)for infrared imaging.Conclusion ICGA and infrared thermal imaging technology have high accuracy in assessing the amputation plane of fingers and can provide a scientific basis for clinical decision-making.
