卡前列甲酯栓联合缩宫素对剖宫产者凝血功能、血红蛋白及产后出血的影响

摘　　要：目的:探讨剖宫产术后卡前列甲酯栓联合缩宫素对产妇凝血功能、血红蛋白(HB)及产后出血效果。方法:回顾性选择2022年7月-2023年10月本院行剖宫产术产妇145例,依据治疗方案分为对照组72例(予缩宫素治疗)和联合组73例(予卡前列甲酯栓联合缩宫素治疗)。对比两组治疗前、后凝血功能[凝血酶原时间(PT)、凝血酶时间(TT)、部分凝血活酶时间(APTT)、D-二聚体(D-D)、纤维蛋白原(FIB)]、血流动力学变化[血氧饱和度、血红蛋白(HB)、收缩压(SBP)、舒张压(DBP)]、产后不同时间段出血量及治疗后不良反应。结果:治疗后,联合组APTT、PT、TT水平低于对照组,FIB、D-D水平高于对照组(P<0.05),两组血氧饱和度、SBP、DBP无差异(P>0.05),HB水平联合组高于对照组(P<0.05)。联合组产后2h(143.44±19.35ml)、12h(248.74±22.72ml)及24h(328.41±39.96ml)出血量均低于对照组(249.37±30.48ml、331.69±35.88ml、497.74±41.18ml),不良反应总发生率(6.9%)低于对照组(19.4%)(均P<0.05)。结论:剖宫产术后采用卡前列甲酯栓联合缩宫素治疗,可显著减少产妇产后出血量,改善凝血功能,提高HB水平,用药安全可靠。Objective:To investigate the effects of carboprost methyl pessary combined with hysteronin for treating women with caesarean section on their coagulation function,haemoglobin(HB)level and postpartum haemorrhage.Methods:145 women who had undergone cesarean section from July 2022 to October 2023 were selected in this study retrospectively.These women were divided into 72 cases treated with contractions in control group and 73 cases treated with carproterone suppositories combined with contractions in experimental group according to the different treatments.The values of the coagulation function,such as prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT),D-dimer(D-D)and fibrinogen(FIB),before and after treatment,the values of the hemodynamic changes,such as blood oxygen saturation,HB,systolic blood pressure(SBP),diastolic blood pressure(DBP),the postpartum blood loss at different time points and the adverse reactions rate after treatment of the women were compared between the two groups.Results:The levels of APTT,PT,and TT of the women in the experimental group after treatment were significantly lower than those of the women in the control group,and the levels of FIB and D-D of the women in the experimental group after treatment were significantly higher(P<0.05).There were no significant differences in the values of the blood oxygen saturation,SBP and DBP of the women between the two groups(P>0.05).The HB level of the women in the experimental group was significantly higher than that of the women in the control group(P<0.05).The amount of bleeding of the women in the experimental group at 2h(143.44±19.35 ml),at 12h(248.74±22.72 ml)or at 24h(328.41±39.96 ml)after delivery was significantly lower than that(249.37±30.48 ml,331.69±35.88 ml or 497.74±41.18 ml)of the women in the control group.The total incidence of the adverse reactions(6.9%)of the women in the experimental group was significantly lower than that(19.4%)of the women in the control group(all P<0.05).Conclusion:Carbopros
