血清网膜素-1和血管生成素样蛋白2水平与心房颤动病人射频消融术后复发的相关性研究

摘　　要：目的探究血清网膜素-1(Omentin-1)和血管生成素样蛋白2(ANGPTL2)水平与心房颤动(AF)病人射频消融术后复发的相关性。方法选取2021年1月至2022年1月在郑州大学附属洛阳中心医院实施射频消融术治疗的125例AF病人,依据随访结果分为两组:未复发组(n=93)、复发组(n=32);血清Omentin-1、ANGPTL2、白细胞介素(IL)-6、IL-8、肿瘤坏死因子α(TNF-α)水平行酶联免疫吸附测定(ELISA);AF病人术后血清Omentin-1、ANGPTL2水平与各指标相关性采用Pearson分析;多因素logistic分析AF病人术后复发影响因素;采用受试者操作特征曲线(ROC曲线)评价血清Omentin-1、ANGPTL2水平在预测AF病人术后复发中的价值。结果复发组血清IL-6[(3.56±0.73)ng/L]、IL-8[(8.26±2.58)ng/L]、TNF-α[(7.34±2.19)ng/L]、左心房内径(LAD)水平[(37.24±5.23)mm]较未复发组[(1.41±0.26)ng/L、(3.49±1.04)ng/L、(2.67±0.63)ng/L、(31.44±4.16)mm]均显著升高,LVEF水平较未复发组显著降低(P<0.05);与未复发组比较,复发组血清Omentin-1水平显著降低,复发组血清ANGPTL2上水平显著升高(P<0.05);Pearson分析显示,AF病人术前血清Omentin-1水平与炎症因子IL-6、IL-8、TNF-α及LAD均呈负相关,与LVEF呈正相关(P<0.05);血清ANGPTL2水平与炎症因子IL-6、IL-8、TNF-α及LAD水平均呈正相关,与LVEF呈负相关(P<0.05),且Omentin-1、ANGPTL2水平呈负相关;多因素logistic分析显示,高水平Omentin-1为AF病人术后复发的独立保护因素,高水平ANGPTL2、IL-6、IL-8均为AF病人术后复发的独立危险因素(P<0.05);ROC曲线分析显示,血清Omentin-1、ANGPTL2水平预测AF病人术后复发的曲线下面积(AUC)分别为0.88、0.84,两者联合预测的AUC为0.94,效能均优于两者单独预测(Z=1.93、2.10,P<0.05)。结论AF病人术后血清Omentin-1水平显著降低,血清ANGPTL2水平显著升高,与术后复发相关,且两者联合对预测AF术后复发具有一定价值。Objective To investigate the correlation between serum levels of omentin-1 and angiopoietin-like protein 2(ANGPTL2)and recurrence after radiofrequency ablation in patients with atrial fibrillation(AF).Methods A total of 125 AF patients were selected to undergo radiofrequency ablation in Zhengzhou University Affiliated Luoyang Central Hospital from January 2021 to January 2022,and were divided into 2 groups according to follow-up results:no recurrence group(n=93)and recurrence group(n=32);enzyme linked immunosorbent assay(ELISA)was applied to measure serum levels of Omentin-1,ANGPTL2,interleukin(IL)-6,IL-8,and tumor necrosis factor-α(TNF-α);Pearson was applied to analyze the correlation between serum levels of Omentin-1 and ANGPTL2 and various indicators in patients with atrial fibrillation after radiofrequency ablation;multivariate Logistic regression was applied to analyze the influencing factors of recurrence in patients with atrial fibrillation after radiofrequency ablation;receiver operating characteristic curve(ROC curve)was applied to evaluate the value of serum Omentin-1 and ANGPTL2 levels in predicting recurrence in patients with atrial fibrillation after radiofrequency ablation.Results The serum levels of IL-6[(3.56±0.73)ng/L],IL-8[(8.26±2.58)ng/L],TNF-α[(7.34±2.19)ng/L],and left atrial diameter(LAD)[(37.24±5.23)mm]in the recurrent group were obviously higher than those in the non recurrent group[(1.41±0.26)ng/L,(3.49±1.04)ng/L,(2.67±0.63)ng/L,(31.44±4.16)mm],while LVEF level was obviously lower than that in the non recurrent group(P<0.05);compared with the non recurrent group,the serum Omentin-1 level in the recurrent group was obviously reduced,while the serum ANGPTL2 level in the recurrent group was obviously increased(P<0.05);Pearson analysis showed that the serum level of Omentin-1 in patients with atrial fibrillation before radiofrequency ablation was negatively correlated with inflammatory factors IL-6,IL-8,TNF-α,and LAD,and positively correlated with LVEF(P<0.05);the level of serum AN
